About the Author:

Richard Larkins, MD (Melb.), BS, PhD (Lond.), FRACP, FRCP, is James Stewart Professor of Medicine at the Royal Melbourne Hospital. Richard Smallwood, MD, BS, FRACP, FRCP, is Professor of Medicine, University of Melbourne at Heidelberg Repatriation Hospital. Both authors have published widely.
