9780702043185: Therapeutic Stretching, Towards a Functional Approach

Prepared by an international authority, Therapeutic Stretching: Towards a Functional Approach offers a highly accessible account of the latest information regarding the role of therapeutic stretching in the management of restricted range of movement (ROM) disorders.

Richly illustrated throughout, this exciting new volume covers the physiological basis of stretching, explores its classification and examines what is normal and abnormal in terms of ROM. Chapters then explain the processes that are associated with ROM loss and recovery as well as what makes stretching effective. The experience of pain, sensitization and pain tolerance in relation to stretching and ROM recovery are also discussed.

Embracing the latest scientific research, this revolutionary book debunks many traditionally accepted stretching theories and associated techniques and supplants them with a more robust, scientifically valid system of practice.

Accompanied by a range of practical on-line videos and over 150 photographs, this volume will be ideal for all manual and physical therapists, sports and personal trainers, and athletes who require special movement ranges.

Book Description:

"The topic of stretching within healthcare, medicine and the sporting world has come under much scrutiny and debate over the last decade. Many approaches have been researched and often a mixed bag of results has been found when it comes to utilising different approaches for dysfunctions and conditions, often leading to a fairly negative outlook for physical therapies and the value of stretching. Understanding and applying the current evidence base means the clinicians must constantly accept and incorporate these findings and transform their clinical practice to suit. This, as Eyal Lederman discusses, is not an easy task especially as many stretching techniques have not changed for decades or even centuries.

Eyal Lederman has written this book with the aim of helping clinicians to incorporate the evidence for stretching, and showing the direction and changes that we as clinicians may need to consider. It is written from a several sources; research and evidence from biomechanics, biomedicine, health studies, motor control, psychology, cognition and behaviour, but also from his own professional experience. A functional approach is detailed and a number of highly detailed examples show the clinician how to apply these techniques clinically.

The book is split into 14 chapters. The book starts with the difference between therapeutic and functional stretching. This is then followed by a detailed analysis of what is normal range of motion (ROM), what is dysfunctional and the causes of this and the subsequent therapeutic potential for rehabilitation. It then discusses the specificity, adaptation, overloading conditions, motor control inputs, exposures and scheduling of what makes stretching effective. These sections discuss identifying behaviours as one of the main driving forces for adaptive changes. It is packed with critical analysis of research, stretching theories and physiology, making the reader gain both a breadth and depth of knowledge that challenges and directs their practice dependent upon the task and stimulus required.

From chapter nine onwards, the book directs the reader towards Lederman's functional approach to individuals who experience a ROM loss from a condition or dysfunction. It brings into the appraisal the impact of pain, sensitisation and pain tolerance to stretching and range of motion recovery. It moves further into a biopsychosocial approach as the psychological considerations in ROM management are addressed. Chapter 12 integrates all of this information from all previous chapters into a functional approach in therapeutic stretching. This approach focuses on using a goal orientated task specific the patients needs allowing them to regain functionality. The book finishes with both a demonstration of this approach with details to a further website, and a summary chapter to finish.

A high level of critique is demonstrated throughout the book which appraises the literature very well. It brings together the umbrella term of stretching and allows the reader to break it down into understandable chunks within the multidimensional process of clinical reasoning. It places focus on redefining stretching into a ROM recovery process that is multi-factoral and highly influenced by the patient's environment. This alongside Lederman's own experiences allows the reader to gain a deeper understanding of the use of therapeutic stretching. Furthermore, the inclusion of additional videos online adds to the value of this book.

This is a highly enjoyable and well presented book that I recommend for any clinician from student to experienced practitioner. It is suitable for all physiotherapists, manual therapists, sports physiotherapists/therapists, strength and conditioning coaches, sports scientists, athletes and patients who would like to understand, recover and improve their range and ease of movement."
