9780765702562: When the Body Is the Target: Self-Harm, Pain, and Traumatic Attachments

In this comprehensive and insightful work, Dr. Sharon K. Farber provides an invaluable resource for the mental health professional who is struggling to understand self-harm and its origins. Using attachment theory to explain how addictive connections to pain and suffering develop, she discusses various kinds and functions of self-harm behavior. From eating disorders to body modifications such as tattooing, Dr. Farber explores the language of self-harm, and the translation of that language and its psychic functions in the therapeutic setting. She tells us, 'When the body weeps tears of blood, we need to wonder what terrible sorrows cannot be spoken.' Brilliantly illustrated with rich clinical material, this book offers a practical approach to the diagnosis, assessment, and treatment of the increasing number of patients whose emotions are expressed through bodily harm. The challenges of working with patients who tend to view the world of relationships in terms of predator and prey are clearly explicated and the stormy countertransference responses that threaten to destroy the treatment are given a full hearing. Finally, she shows how the attachment relationship formed in treatment can repair the traumatic attachment in mind, body, psyche, and soul, and can serve as the cornerstone of therapeutic change. From the Author:

Interview with Dr. Sharon Farber in The Psychotherapy Book News: A Journal of Essays and Reviews

Q: Your concept of the inner predator is intriguing. Can you tell us more about it and how you find it so valuable in working with patients who harm themselves?

Dr. Farber: It's the complement to the "inner child" concept that has been embraced by the public. The "inner predator" is that destructive part of the self that resides in all of us, coexisting with the innocent child who needs to be cared for and protected. This is about the human struggle between good and evil, devil and angel. The safety/danger dialectic is paramount in the mind of the person who harms himself in that he does not feel safe either from predators or demons in his unconscious or from real predators in his environment. The inner predator grows bigger and stronger each time a perverse part of the self allows the healthy part of the self to collude with and be taken over by the destructive part. It is like feeding raw meat to the hungry beast and finding that it does not satisfy but stokes the blood lust for more.

What I find exciting about using the inner predator concept clinically is that it helps patients begin to acknowledge that the beast inside them is not some foreign force that needs to be purged, but is as much a part of them as the healthier part that wants to keep them from hurting themselves. Working like this can help the patient confront her disavowal of her destructive wishes and begin to acknowledge and take responsibility for them.
