That it is comprised of occasion, speaker, and topic is a gross oversimplification. It also includes challenge, anxiety, emotion, fear, responsibility, faults of memory, and instants of pride. Out of the circumstances arise an increase in heart rate, a change in blood pressure, an abnormal pattern of breathing, a noticeable build up in perspiration, and an ongoing evaluation. For students this may be merely a grade or perhaps a series of evaluative remarks, possibly addressed both to the speaker and the other participants, the audience. It may entail a replaying of a record of the speech, indeed a videotape. Most important is the lasting impression that remains with all of the participants. He has outlined a composition around a central idea or thesis. He has marshaled evidence, details. He has framed an opening paragraph. He has been admonished not to give an essay, but to strive for audience contact, interpersonal communication. He makes his audible approach through his vocabulary and accompanying phonology. Under the tension, the speaker repeats; he adds meaningless vocalizations in periods that might logically be pauses. There are slips of the tongue. Typically, after reading the recommended materials, the student returns to request more detailed accounts of how clinicians 'think' about particular cases. The general lack of such materials in the behavior therapy Iiterature led to the formation of the present volume. Throughout much of the behavior therapy literature, one gets the impression that most cases seen present circumscribed and straight fmward psychological problems. In my experience, such cases are rare. Accordingly, the present volume was designed to cover more complex problemssuch as sociopathy and paranoid personality. Thesedisorders are rarely discussed in the behavior therapy Iiterature but nonetheless seem to appear regularly in clinical settings. The cases presented in this book are descriptions of patients seen clinically by the editor or by the contributors. Work an this text began while I was a faculty member at Vanderbilt University and took several years to complete. As the contributors would attest, the task I set out for them was atypical and often difficult. The complexities involved in articulating how one conceptualizes a case are numerous and may help to explain why there are so few sources available on case formulation. Like volume IX, this one focuses on one of the subgroups to which we are committed--the neonates' special fluid and electrolyte requirements. This volume has more on blood pressure and renal function and looks at the hormonal regulators. There is greater depth about intoxications and nephrotic agents, congenital disorders and mineral metabolism. The exchanges were stimulating and the controversies were brought out without need of much of my usual prodding. At Julie Ingelfinger's suggestion, at the end of each panel discussion I have added a comment to highlight the main points as I see them. Otherwise, the format remains as in past editions: the papers given related to the four major topic areas, each followed by panel and registrant discussion. Although the transcription is almost verbatim, you will not find the names of the discussants, purposely omitted to ease my editorial work and to encourage everyone to speak candidly. Some of the questions and answers are those submitted to the panelists after the sessions, incorporated here by request. Buch. Condizione: Neu. Neuware - The behavior of the kidney in normal pregnancy, as well as in complicated pregnancy, is a very interesting, but still in many ways an unknown topic in renal medicine. It is undoubtedly difficult to determine, even in normal women, the behavior of renal hemodynamics throughout gestation, since the fear of impairing a new life (i.e., the fetus's life) will limit, for ethical reasons, the use or the frequent repetition of diagnostic tests on the mother. On the other hand, the study of complicated pregnancy even for diagnostic purposes (for planning adequate treatment), except in a few countries that are known for the advanced health education of the population, has to face serious difficulties. First of all, pregnant women usually seek the help of an obstetrician when gestation is already in an advanced stage. This makes it difficult to determine when and how asymptomatic signs of any disease discovered during pregnancy have first occurred. A second difficulty is that frequently the patient does not know whether a given disease has preceded pregnancy. Pregnancy is a condition of young women, and a young woman frequently has never seen a physician; thus, no urine analysis or blood tests have been performed before the gestation. Not infrequently, even blood pressure has never been measured. This will make it difficult to classify hypertension discovered in late pregnancy as pregnancy-induced hypertension or as chronic hypertension in pregnancy.

Gains with cytotoxic chemotherapy have been much more modest. Of the approximately 900,000 newly diagnosed cases of cancer each year, 50010 result in death of the patient. Although the term metastasis was first coined by Recamier in 1829, only in the past ten years have there been intensive scientific investigations into the mechanisms by which tumor cells metastasize. What has emerged is a complex process of host-tumor cell interactions which has been termed the metastatic cascade. Due to the complexity of the metastatic process, the study of metastasis is multifaceted and involves elements of such areas as differentiation, en zymology, genetics, hematology, immunology, membrane biochemistry and molecular biology. The major objectives of this book were to present the most recent advances in our understanding of how tumor cells metastasize to secondary sites by the leading experts in the biology of tumor invasion and metastasis. We hope that this book will lead to new concepts for the treatment of subclinical metastatic cancer. The chapters in this book address both the basic science of metastasis and potential clinical therapies directed toward interruption of the metastatic cascade or toward eradication of subclinical metastases. The last major monograph on the subject (Ewer, 1968) was published sixteen years ago, and there is a need for this information to be examined in terms of modern sociobiological theory. My approach throughout is evolutionary and is therefore directed strongly towards research which throws light on the ways in which mammals behave in their natural environments. I have tried to cover as wide a range of mammalian species as possible, although, in some cases, the only data available were obtained from captive individuals. The coverage of this book is not a reflection of the volume of literature published on different species, as I have tried to avoid undue emphasis on the social behaviour of primates and laboratory rodents. I have made scrupulous efforts throughout to avoid an anthropomorphic approach to mammalian behaviour. Terms such as 'strategy', 'evaluation' or 'choice' do not therefore imply conscious planning, but are used neutrally in the way in which they would be applied to a chess-playing computer. In the case of mammals, the programmer was natural selection. While I am fully aware that human beings are mammals, any detailed consideration of human social behaviour lies outside the scope of this book. A single exception was that dealing with a description of spin-resolved photoelectron spectroscopy at energies up to 40 eV (31 nm). This work was done using circularly polarized radiation emitted above and below the plane of the circulating electrons in a synchrotron ring. The device at BESSY (West Germany) in which the experiments were carried out seems to be the only one presently capable of providing circularly polarized radiation in the X--ray through vacuum ultraviolet energy range. A much more intense source is needed in this range. A possible solution was proposed which could provide not only circularly polarized photons over a wide energy range, but could in principle modulate the polarization of the beam between two orthogonal polarization states. Realization of this device, or an equivalent one, would be a vital step towards the goal of determining all components of the Mueller matrix for each spectroscopic experiment. A variety of theoretical treatments are presented describing the different phenomena emerging from the interaction of matter and polarized radiation in a wide range of energies. The difficulty of the problem is attributed to the large number of often conflicting factors that affect the routing quality. Traditional techniques have approached routing by ignoring some of these factors and imposing unnecessary constraints in order to make routing tractable. In addition to the imposition of these restrictions, which simplify the problems to a degree but at the same time reduce the routing quality, traditional approaches use brute force. They often transform the problem into mathematical or graph problems and completely ignore the specific knowledge about the routing task that can greatly help the solution. This thesis overcomes some of the above problems and presents a system that performs routing close to what human designers do. In other words it heavily capitalizes on the knowledge of human expertise in this area, it does not impose unnecessary constraints, it considers all the different factors that affect the routing quality, and most importantly it allows constant user interaction throughout the routing process. To achieve the above, this thesis presents background about some representative techniques for routing and summarizes their characteristics.
