This update of the 1999 edition reviews changes prompted by the same-year Institute of Medicine report, To Err is Human: Building a Safer Healthcare System, stating that about 7,000 patients die each year from medication errors. Cohen (Institute for Safe Medication Practices, Huntingdon Valley, Pennsylvania), a pharmacist expert on such errors, introduces 23 chapters examining the scope of the problem, and traditional and new approaches to an issue that is costly in human and economic terms. Error detection approaches discussed include collaboration, proactive risk assessment, incident reports, high-alert drug designations, and computer-assisted monitoring.
