The construction industry is facing the significant problems of high cost of project delivery, bad financial performance and inability to deliver value to customers on time. A major factor contributing to this setback is rework. Rework is defined as the unnecessary effort of redoing an activity that was inaccurately done the first time. In this research work questionnaire survey having various reasons of rework was conducted, Survey was carried out in Surat City south Gujarat Region. From the various Literatures 48 Causes of rework were finalized under 7 Major Group which was checked for their impact in construction using IMPORTANCE INDEX (IMPI) method and top most crucial causes were used to develop FIELD REWORK INDEX (FRI). The FRI score between 14 to 30 was approaching stage, 30 to 45 was normal vigilance stage and 45 to 70 was rework alert stage. After FRI questionnaire was distributed in 11 building construction sites out of 3 sites (27.27%) sites are having success approach, 8 sites (72.73%) are having normal chances of rework and 0 sites are having high alert.
