Cancer Treatment and Research is a series of authoritative volumes which aim to meet this need. It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal subscription. We have approached the problem in the following fashion. First, by dividing the oncology literature into specific subdivisions such as lung can cer, genitourinary cancer, pediatric oncology, etc. Second, by asking emi nent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities, basic biology, and more.

The development of the technique of continuous ambulatory peritoneal dialysis (CAPD) and continuous…cycling peritoneal dialysis (CCPD) has markedly changed the approach to children requiring dialytic therapy. The availability of these techniques has facilitated prolonged dialysis in infants and has for the first time given pediatric nephro logists in many areas of the world an opportunity to consider dialysis in chil dren afflicted with ESRD. I have enlisted the collaboration of colleagues from Europe, South America, Canada, and the United States in compiling this multidisciplinary text, which hopefully contains the most up-to-date, comprehensive information regarding the use of CAPD/CCPD in children. It is my hope that every nephrologist (pediatric and adult); nephrology nurse (pediatric and adult); nephrology tech nician, or allied health professional dealing with children who require these therapeutic modalities will be able to resolve immediately any confounding clinical or technical issues that arise by using the information contained in this text. Demographic data on the use of CAPD/CCPD in children in Europe is provided from the EDTA Registry and in the United States from the National Peritoneal Dialysis Registry. The particular problems encountered in the use xiii xiv Preface of CAPD in children in developing countries is detailed by Dr. Grunberg and his colleagues in Uruguay.

Andreucci of keeping alive patients in terminal chronic Initially created with the purpose renal failure, dialysis has undergone improvements in methodology, and its final goal has become complete health rehabilitation and optimization of the quality of life of chronic dialysis patien…ts. To achieve this, many investigators have attempted to increase dialysis efficiency and at the same time shorten dialysis time. Their main concern was, obviously, patient safety: the Latin proverb 'primum non nocere' is still valid all over the world. Thus, when clinical observations of the first patients on regular dialysis therapy suggested an inverse relationship between duration of dialysis sessions and severity of peripheral neuropathy, long and frequent dialysis sessions were considered the only way to prevent the catastrophic consequences of nerve damage and underdialysis syndrome. It was then, in 1971, when dialysis duration was 8- 12 hours per session, that Vincenzo Cambi started a 'short dialysis' trial, i. e. , 4 hours 3 times weekly or 3 hours every second day. For the first time, dialysis was shortened from 24-36 hours weekly to 10. 5-12 hours weekly [1, 2]. In 1971 I was still at the Parma University Hospital. We had both just returned from the United States, and Dr. Cambi was responsible for the dia lysis unit.

The aim of these symposia is to pro…vide a supportive atmosphere for the development of new concepts as well as for the presentation of high quality scientific data. This is reflected in the chapters presented here. Many of the chapters put forward new hypotheses which will be tested in the clinic or laboratory in the next few years. In choosing the subjects for discourse, we preferred to tackle areas where controversy existed, and in this book we have grouped the chapters around discussion themes. The relative merits of mammography and physical examination in the detection of small lesions within the breast and the natural history of these lesions were important topics at the symposium, and the results of early detection studies will have a major effect on health care costs. We have devoted a large section of this book to this subject. The mechanism of growth and response of endocrine sensitive tumors and current concepts of the statistics and cost-benefit assessment of adjuvant chemotherapy trials comprise two other sections of the book where original and stimulating ideas were presented.

Shortly after Thomson and Clausius enunciated their versions of the Second Law, Clausius, Maxwell, and Boltzmann began actively pursuing the molecular basis of thermo d…ynamics, work that culminated in the Boltzmann equation and the theory of transport processes in dilute gases. Much later, Onsager undertook the elucidation of the symmetry oftransport coefficients and, thereby, established himself as the father of the theory of nonequilibrium thermodynamics. Com bining the statistical ideas of Gibbs and Langevin with the phenomenological transport equations, Onsager and others went on to develop a consistent statistical theory of irreversible processes. The power of that theory is in its ability to relate measurable quantities, such as transport coefficients and thermodynamic derivatives, to the results of experimental measurements. As powerful as that theory is, it is linear and limited in validity to a neighborhood of equilibrium. In recent years it has been possible to extend the statistical theory of nonequilibrium processes to include nonlinear effects. The modern theory, as expounded in this book, is applicable to a wide variety of systems both close to and far from equilibrium. The theory is based on the notion of elementary molecular processes, which manifest themselves as random changes in the extensive variables characterizing a system. The theory has a hierarchical character and, thus, can be applied at various levels of molecular detail.

Although the findings have been exciting and have revealed unique properties of neural cells, considerable concern continues to be expressed regarding the… significance of in vitro findings in terms of their applicability to in vivo biological events. To examine this issue further, a group of scientists pre sented and discussed their findings at a conference sponsored by the Institute of Developmental Neuroscience and Aging held in Crete, Greece, 26-29 May 1985. The conference was cosponsored by the University of Crete and was generously supported by the Ministry of Research and Technology of Greece, Tourism Organization of Greece, and also Sandoz and FIDIA. The Directors of the Institute of Developmental Neuroscience and Aging are indebted to these Institutions for their support. For the success of this conference, the Directors owe much to Drs. Eleni Fleischer-Lambropoulos and Yiannis Tsouderos, who spent countless hours in making arrangements so that the participants would have not only a scientific, but also a unique cultural, experience. Several chapters of this book focus on the complex phenomena of neurogenesis and gliogenesis, and the modulation of neuronal differentiation. The concept that neuronal differentiation has both genetic and epigenetic components is documented by elegant studies using both in vitro cultured cells and neurons transplanted in vivo.

He knew perfectly well how to persuade the unwilling and to disenchant the illusionist. Sometimes a visionary himself, he would not pursue his ideas unless there was a chance of success. As innovative Prof. P. De Somer was in providing to this University a new face, or should I say facelifting, as international is his reputation as the founder, and, since its inception, only director, of the Rega Institute. Built now more than 30 years ago, the Rega Institute has re mained one of the world's leading centers in microbiological research.

The original printing of 300 copies by the DNA Press was followed shortly… by a small second printing to meet the demand by readers who heard of the book from the primary recipients. This supply was also quickly exhausted, to researchers and practicing engineers outside the DNA community and to academics who wanted to include the material in courses on elastic and plastic stability of structures. Commercial publication by Martinus Nijhoff Publishers was therefore undertaken to meet the needs of this broader community. The objective of the book was to gather into a cohesive whole material that had been published in reports and the open literature during the two decade period. In the process of knitting this material together, a substantial amount of new work was done. The book therefore contains many new results never published in the open literature.

The I -measure is shown to differ from other homology and homotopy measures in that it is calculable with respect to most of the important geometric constructions encountered in algebraic topology. This approach provides a new method of treatment and leads to various new results. In particular, an axiomatic system of I -measure is formulated, quite different in spirit from the usual Eilenberg-Steenrod axiomatic system for homology, and giving at the same time an algorithmic method of computation of the I -measure in concrete cases.
