This has been brought about by technical developments in imaging, notably, three-dimensional ultrasound and magnetic resonance imaging (MR…I), which allow high anatomical resolution and tissue differentiation to be presented in a most usable fashion. Three-dimensional endosonography in anorectal conditions and MRI in anal fistula are two obvious developments, but there are others, with dynamic st- ies of the pelvic floor using both ultrasound and MRI coming to the fore. This atlas provides an easy way to gain a detailed understanding of imaging in this field. The atlas is divided into four sections covering the basic anatomy, anal/perianal disease, rectal/perirectal disease and functional assessment. One of the difficulties with developing an atlas is to strike the right balance - tween text and images. Too much text and it is not an atlas; too little text and the - ages may not be understood. The editors of this atlas are to be congratulated on achi- ing an appropriate balance. The images are all that one expects from an atlas, and the diagrams are excellent. The commentaries at the end of invited chapters are a valuable addition, placing what are relatively short, focussed chapters into context. They add balance and depth to the work and are well worth reading. 256 pp. Englisch.

Prior to the ea…rly studies on its th anatomy and physiology in the 18 century, the thymus was believed to perform - usual and curious functions such as purification of the nervous system, providing a protective cushion for the vasculature of the superior mediastinum, fetal nouri- ment, or more spiritual roles such as being the seat of the soul, among others. D- th ing the 19 century important anatomical/physiological studies took place focusing on the role of the thymus in pathological conditions. However, it was not until the th middle of the 19 century that a more comprehensive analysis of the role of the thymic gland and its role in pathogenesis began to emerge. Currently, while the knowledge gained on the diverse aspects of the thymic gland has furthered our understanding of its role in a gamut of processes, more knowledge is still being sought, and by no means is a full understanding of the gland's physi- ogy and pathology complete. Different aspects, including its purported endocrine function, its association with other autoimmune diseases like multiple sclerosis, rheumatoid arthritis, and lupus erythematosus, among others, are under evaluation and research. 292 pp. Englisch.

Although the majority of surg…eons today are unfamiliar with the his- ry of early minimally invasive surgery developments, it is important to realize that the advent of laparoscopy led not only to new surgical te- niques, but also to a change in the doctrine of medical care, by streng- ening the concept of minimal invasiveness. This is particularly the case for biliary lithiasis, for which laparoscopy has provided major benefits in terms of both diagnosis and surgical tre- ment. However, our understanding of the etiology of this disease, as well as the availability of effective medical therapies, have remained subst- tially unchanged over time. The application of the innovative approach has not been, in routine practice, without problems: between 1990 and 2000, the rate of serious complications from laparoscopic cholecystectomy were four times higher than those observed with open surgery. Even initial attempts at lapa- scopic treatment of biliary common bile duct stones resulted in excessive complications and associated medico-legal problems. Nonetheless, today, most indications have been stated, techniques have been standardized, and a new aspect to the treatment of biliary lithiasis has appeared. 536 pp. Englisch.
