The chapter on the management of skin d- eases also gives the details of topical, systemic, and the phy- cal modalities used in treating skin disease. Uncommon skin diseases are only mentioned where required. The chapter on cutaneous manifestations of systemic diseases will help the general practitioner to correlate the cutaneous signs of the common medical problems seen by them. Emphasis is laid on the bacterial, fungal, and parasitic disorders that are pre- lent in tropical countries. We have included the common d- eases of other continents, as the general practitioner especially of developed countries has patients from all over the world. Congenital and hereditary disorders are discussed with the corresponding chapters, which makes it easier for the reader to remember. I myself will never forget thumbing through an older edition of the Prim…er as a second-year resident, while waiting to review a perplexing patient with my tutor. Fortunately the tutor was r- ning late with his own patients, so I had time to fip through the book - then much thinner - a couple of times. While turning the pages, per- ing the features of those diseases whose names were still exotic to me, and considering my patient's history of conductive hearing loss and p- monary nodules, a light went on when I stumbled eventually on a part- ular chapter. I still remember the jaw-dropping efect on my tutor of my announcement then that I had a patient with Wegener's granulomatosis. I think I became a rheumatologist that very moment! Subsequent editions of the Primer have sufered from the inevitable 'obesity creep,' making it an outstanding reference textbook but virtually impossible to fip through quickly while awaiting one's tutor, and even more difcult to slip into the pocket of a white coat to carry on rounds. Major contributions…to this volume are derived from the work done in the PROVIDE project (working on sustainable urban infrastructures in cities of the Lake Victoria Basin, East Africa), the DESAR project (research and pilot projects in Decentralized Sanitation and Reuse, the Netherlands), and among others within NETSSAF (large scale implementation of sanitation in Africa), and EcoSan networks. The major milestone for this book to emerge was however the IWA Sanitation Challenge Conference of May 2008 in Wageningen, the Netherlands where all the authors of this book presented their papers. The conference was organized by a consortium of sanitation specialists at Wageningen University's Environmental Policy Group (the editors) and the s- department of Environmental Technology, LeAF (Lettinga Associates Foundation) and Wetsus (Center of Excellence for Sustainable Water Technology in the Netherlands). It was a unique event as it enabled a truly multi-disciplinary approach in discussing Sanitation Challenges in North and South with social and political scientists, natural scientists, environmental engineers and practitioners in one s- entific conference.
