The methods used in factory laboratories to prevent such waste are largely empirical. The properties of the surface, the nature of the physicochemical processes that take place on it, and the role of environmental factors still remain obscure. A major task of the semiconductor industry is to learn to control the properties of the surface.

The response to this volume was… superb, indicating a tremendous interest in basic growth regulatory mechanisms governing breast cancer and controlling its malignant progres sion. In the two years since its publication, much new and exciting in formation has been published and the full interplay of regulatory mechanisms is now beginning to emerge. We have divided this book into four sections that we hope will unify important concepts and help to crystallize areas of consensus and/or disagreement among a diverse group of basic and clinical scientists working on the disease. The first section is devoted to studies on oncogenes, antioncogenes, proliferation, and tumor prognosis. The first chapter, by Sunderland and McGuire, introduces the characteristics of breast cancer as studied by patho logists to establish prognostic outcome. Of particular interest is a new proto oncogene called HER-2 (or neu), which is rapidly becoming accepted as a valuable new tumor marker of poor prognosis. The second chapter, by Lee Bookstein and Lee, introduces the best known antioncogene, the retinoblas toma antioncogene, whose expression is sometimes lost in breast cancer. Malignant progression appears to be influenced by the balance of proto oncogene and antioncogene expression.

It must also be realised that there are hazards in withholding some therapeutic agents from pregnant women, and these hazards have to be weighed against the dangers of indis criminate drug therapy. It is often very difficult to obtain relevant information about any given compound in relation to its use in pregnancy, and it is therefore appropriate to provide a handbook which brings together information about a wide variety of drugs in a form which allows ready access for the practising clinician. The focus of the book is the 'old' Community Health Ser…vices: those transferred to Area Health Authorities from local authorities in the 1974 re organization of the NHS. These diverse services, while grouped together, had little objectively in common, occupying, as they do, a hazy middle ground between hospital and Family Practitioner Services. However, since 1974 there have been a number of major devel opments which have opened opportunities for change and devel opment in these services. These include: the resurrection of concern with 'Public Health'; the attempted closure of large mental illness and mental handicap hospitals and the development of 'Com munity Care'; the introduction of General Management; and the implication for health and local authorities of the White Papers 'Caring for People', 'Promoting Better Health' and 'Working for Patients'. Traditionally, Community Health Services were seen as low status and a professional dead-end. This, in tum, has led to a rather uneven body of literature. The growth of general management has led to a demand for a more coherent, management-orientated literature. NHS nursing homes have been created, enthusiasts have developed new initiatives, and attempts have been made to improve att…itudes. The recent White Paper on Community Care is likely to accelerate the transfer of large numbers of elderly patients from long-stay hospital beds to private-sector accommodation: a move often accompanied by much anxiety about standards of care. Although Health Authorities visit private nursing homes and apply the National Association of Health Authorities' standards to them, anxieties continue. Unfortunately for the patients who remain in hospital, many Health Authorities seem reluctant to apply these same standards to their own long-stay departments, since many would fail abysmally. The 1987 annual report of the Health Advisory Service (see Chapter 3) presents a damning indictment of the care given to old people: A recent review of twelve consecutive HAS Reports on services for older people in hospitals shows that long-stay wards consistently offered environments which were unable to offer privacy, homely surroundings, personal space and possessions or adequate furniture. In the twelve districts there was not one comprehensive personalised clothing service. Half of the reports commented on the lack of effec tive management of continence. I was giving the third project tutorial of the afternoon. Nevertheless, I noticed that some of the material was common to all three students. Unfortunately, the basics were precisely those topics which I considered essential to the success of any project. What's more, they dealt with the sort of material which wouldn't, on the whole, have occurred spontaneously to many students, and so it was a necessary part of my job to go through them. Tangible evidence of this lies in the publication of the Journal of V…oice in the United States, and in the UK the formation of the British Voice Association. This organization has attracted an increasing membership from professionals involved in all aspects of voice care and use - actors and singers, laryngologists and speech therapists, teachers and phoneticians. The Association holds regular study days, holds an annual two-day symposium, and publishes a Newsletter which attracts entries from this broad spectrum of professionals. We have also seen an increase in the number of specialist voice clinics, and in the two final chapters in this book a contrast is presented between such a specialist setting and the more typical clinic that operates in the majority of general hospitals. This last chapter now contains a breakdown of voice referrals over an eight-year period, which must represent a unique published study in this country. There still remains, however, little research into the management of voice disorders. There is clearly a need for more efficacy studies into specific treatment methods, and the single case-study designs developed in the field of aphasia would seem to be appropriate here. 412 pp. Englisch.

Most of these amputees are elderly and their life expectancy is short, so it is important that the treatment and rehabilitation that they receive is informed, appropriate, efficient and swift to enable them to return successfully to life in the community for their remaining years. Management of this group of patients has proved to be successful only if a multidisciplinary team approach is adopted. Until recently in the UK, this approach sadly has only been implemented by a few centres. However, with the publication of the McColl report into the prosthetic and wheelchair service in 1986, interest in the care of the amputee is growing throughout the country. This book covers all aspects of amputation from disease and diagnosis to rehabilitation and community discharge with emphasis on the man agerneut of the largest group, the vascular lower limb amputee.
