However, since the mid-1980s there has been a growing in terest in the application of community care principles to mentally disordered offenders. A new set of attitudes have…developed which have enabled the mentally disordered offender to emerge from relative institutional obscurity to a much higher profile in the community. Although numerically small in relation to the general psychiatric population, forensic patients tend to attract the public' s attention by virtue of their greater propensity for troublesome behaviour. Such a group needs expert community support in order to maintain an accepta bie public face. Unfortunately these developments have outstripped the creation of training opportunities for community staff charged with their support and supervision. Most community practi tioners have had to learn 'on the job' with inadequate support and supervision from senior staff, who of ten have less direct experience of working with this client group than themselves. Just such a situation prompted the authors, in 1990, to establish a module on 'Working with Mentally Disordered Offenders in the Community' as part of an MA in Social W ork at the University of Reading. As the course developed it became clear that the topic and content had equal application to other community professionals working in the field.

It is to be expected that such mechanisms are reflected in the neural dynamics: Neurons or neuronal assemblies which pertain to one object may fire, possibly in rapid bursts at a time. Such firing bursts may enhance the synaptic strength in the corresponding cell assembly and thereby form the substrate of short-term memory. However, we may well become aware of two different objects at a time. How can we avoid that the firing patterns which may relate to say a certain type of move ment (columns in V5) or to a color (V 4) of one object do not become mixed with those of another object Such a blend may only happen if the presentation times be come very short (below 20-30 ms). One possibility is that neurons pertaining to one cell assembly fire syn chronously. Then different cell assemblies firing at different rates may code different information.

This programme developed rapidly due to the enthusiastic response of those in the field; and by 1992 the total value of the programme was in excess of Pds. 2 million and currently totals over Pds. 3. 2 million. It comprised a cohesive matrix of grants commissioned throughout the United Kingdom for work in identified areas which, taken in conjunction with existing grants, formed a comprehensive programme of much-needed studies and research in this field. The whole was to be underpinned by a major study planned and financed by the Trust but conducted by the Royal College of Physicians to survey the existing services being provided by some 180 health authorities. This study, together with concern for the handicapped schoolleaver, information about the availability of disability services and the care of mild/moderate head-injured persons, formed the prime initial thrust of the programme. In the case of head injury, the Trust was particularly concerned that there was discontinuity of care for many patients and little evidence as to which treatments were effective. Once patients had stabilized in hospital they were discharged into the community under the care of their general practitioner often without a continuing responsibility from the hospital. 288 pp. Englisch.

I am delighted that this work competently deals with… nursing's dilemmas about its therapeutic role over a crucial period in its history, especially the effects of major structural change on the profession between the period from the implementation of the Salmon and Mayston Reports up to the present time and Project 2000. The picture is in Dr Bowman's opinion not a very happy one from the viewpoint of the profession. Some of the changes that have taken place have involved some loss of professional cohesion and were independent of the organizational framework of the National Health Service. Nevertheless, it is disturbing that nurses appear during the period covered to have Ilad a quite inadequate input to the shaping of Health Policy. There are already signs that the nursing profession has begun to absorb the warnings ci ted in Dr Bowman's study. However, in the wake ofthe National Health Service reforms and the implementing of a business culture in the National Health Service, nurses may find the message of this book only too familiar. That does not for one moment mean that the nursing input to corporate strategy and general direction should be defensive or reactionary. To take account of these, much needs to be added to this history. Late 1989 saw the publicat…ion of the double volume set of studies, Effective Care in Pregnancy and Childbirth, in which all the then existing evidence on all the associated procedures was considered and evaluated by well-informed and impartial authors representing many countries. This informative collection has since been followed by a flow of single reports of new research findings about specific subjects within the field. To incorporate the new material has involved, in particular, a consider able enlargement and rearrangement of the text and reference lists for Chapters 3 and 4, which deal with antenatal and intranatal care. Then in 1990-1 the House of Commons Health Committee, under its chairman Nicholas Winterton, undertook a further enquiry into the maternity services in Britain. A wide range of people concerned as pro viders or users of the service, as well as researchers concerned to find out how well the service was meeting needs, chose to submit testi monies, written and oral.
