This item is printed on demand - it takes 3-4 days longer - Neuware -Stress-induced myocardial ischemia is a frequent manifestation of coronary heart disease, and sympathetic activation is an important precipitating and aggravating factor in such stress induced ischemia. However, the complex interpl…ay between the sympathetic initiation of myocardial ischemia, ischemia-induced alterations in sympathetic neurotransmission, as well as changes in adrenoceptor density and post-receptor signal transduction that can occur during ischemia remains incompletely understood. Not only the activation of myocardial fJ adrenoceptors, but also the activation of coronary IX-adrenoceptors can contribute to myocar dial ischemia. However, the role of fJ-adrenoceptor-mediated increases in contractility relative to heart rate in the initiation of ischemia is not clear, and the significance of IX-adrenoceptor mediated changes in coronary vasomotor tone, as well as the responsible IX-adrenoceptor subtypes are highly controversial. Malignant arrhythmias may be triggered by both IX- and fJ-adrenergic mechanisms. Current research in this field is focussed not only on the underlying physiological and pathophysiological mechanisms, but also on clinical treatment strategies, e. g. , by fJ-blockade, IX-blockade, bradycardic agents and calcium antagonists. Recent findings were presented and future research directions discussed during the 61' International Titisee Conference, held at the Schwarzwald-Hotel, Titisee, March 29-31, 1990 under the sponsorship of the Boehringer Ingelheim Foundation. Although the improvement of surgical technique, extracorporeal circulation, and postoperative management has contrib…uted to a marked reduction of morbidity and mortality, the development of cardiac surgery to its present state would not have been possible without blood substitution by homologous donor blood. Only 20 years ago, open-heart operations required an average of 8 units of blood preserves. The excessive need of donor blood in those early days was mainly due to premature surgical technique, insecure control of anticoagulation, severe blood trauma by extracorporeal circulation, and the lack of retransfusion technologies that would have allowed the reuse of shed mediastinal blood. The introduction of new technologies, such as normovolemic hemodilution, in traoperative autotransfusion, postoperative return of shed mediastinal blood, and predonation of autologous blood has greatly reduced donor blood requirements. At present the majority of routine coronary artery surgical procedures can be performed without any blood transfusion. Blood loss, however, may be considerable in patients undergoing complex valve surgery or reoperations, as they often require several units of transfused blood. Blood conservation has now become an area of major interest for the cardiac surgeon. This increased concern is caused by infectious complications of blood transfusion, in particular hepatitis and, more recently, AIDS. 300 pp. Englisch.

All authors contributed their manu scripts in advance and they arc combined with an edited version orlhe discussions to allow rapid publication. In coronary heart disease the prognosis for patients is largely determined by the occurrence of irreversible ischemic damage to the myocardium. Irreversible eveniS are certainly preceded by a destabilization of a former stable situation. This desta bilization may occur with increasing symptomatology or without apparent warning signs to patient and physician; it may occur over a longer time period or very sud denly. The statemenl, however, holds true that no patient has ever died from stable angina pectoris. Thus, if the prognosis for patients with ischemic heart disease is to be improved, the focus has to be set on recognition and treatment of unstable ischemic syndromes. The contributions and discussions reflect the open-minded and creative atmosphere at the meeting. The systematically structured program of the symposium continued the tradition of the Search for the Causes of Schizophrenia symposia, which were th started in 1986 on the occasion of the 600 anniversary of the University of Hei delberg and which are co-organized with Prof. Wagner Farid Gattaz/Sao Paulo. The aim of these symposia and their proceedings volumes [6,8-10] has been to reflect the state of the art in schizophrenia research at their time, and they have successfully done so. In contrast, the present symposium pursued a more lim ited objective and was of a different type. Moreover, cardiac energetics should be an important consideration in the choice and application of drugs especially in the case of vasodilators, inotropic agents and in cardioprotective measures. Only by considering energetics at the subcellular, cellular, and whole-heart level we can arrive at a better understanding of cardiac performance and ultimately better clinical judgement and drug therapy. The conference was…held as an International Erwin Riesch Symposium un der the auspices of the Deutsche Gesellschaft fUr Herz- und Kreislaufforschung at Spangenberg Castle, March 22-25, 1984. It was made possible through funding by the Erwin Riesch Foun dation and the Bayer AG which deserve our gratitude for their generosity. It may not be fully reflected in this collection of papers that the atmosphere of this meeting was unique. The reason for this resided apparently in the still rudimentary experiences with the adult cardiocyte model that is now widely acknowledged to be very promising. Thus, quite basic inter ests brought together experts from different biological disciplines who in respect to this new mod el were all still 'beginners'. It was noteworthy that such common interests exerted a unifying effect upon otherwise rather separated fields of science. The main task of the years to come will certainly consist in improving isolation and culturing conditions for adult myocytes in order to fully establish the isolated cell model in heart research. 168 pp. Englisch.

We are also pleased - and you can attribute this to my own vanity - with the success and acceptance of the concept of myo…cardial reperfusion and revascularization - specifically selective intracoronary lysis followed either by PTCA or bypass surgery - which we have been pursuing since 1979. But after the dramatic immediate effects of the first attempts, which you have to experience yourself, we did not expect it to be any different. We decided against performing a randomized study in which every patient is catheterized, and thrombolytic therapy only given to some of the patients, for which we have been criticized at times. At the previous symposium in Aachen in 1983 on the topic of thrombolysis for acute myocardial infarction, in the final session several speakers were asked directly about this topic, and replied that if they were patients they would not want to be randomized into the placebo group. In the meantime positive results from large, randomized studies have been recorded, which are presented in this volume. I mention in particular the Western Washington Study from Dr. Kennedy and the Interuniversity Study in Holland. Aachen, a relatively small city with 250,000 inhabitants, provided the opportunity to treat a relatively large patient population with acute myocardial infarction. 240 pp. Englisch.

There have been significant achievements in diagnostic procedures as well as in the medical, invasive,and surgiealtreatment ofischemieheart dise…ase inrecent years. A variety ofdrugs are availablefor the pharmacotherapyofischemieheart disease,par ticularly nitrates, ß-blockers, and calcium-antagonists which are used as mono therapy or in various combinations. However, the selection of patients for a certain treatment, as weIl as the optimization of an individual treatment are still largely empirical. On the other hand, the recent advances in experimental cardiology emphasize the extremely complex and dynamie scenario of ischemic heart disease, involving endothelial damage, coagulation processes, metabolie and morphologie derangements, coronary constrictor mechanisms, blood flow redistribution, arrhythmogenesis, contraetile dysfunction during ischemia and reperfusion, and finally lack or presence of pain perception. Therefore, it appears desirable to close the gap between experimental and clinical cardiology and, thus, to provide a pathophysiologieal basis for rational cliniealdecisions with respect to diagnostic and therapeutic procedures. The idea for this book arose during the preparation of a seminar series on experi mental cardiology, when I found it diffieultto collect the pertinentinformation from textbooks of cardiology, physiology, pathology, and pharmacology, as well as from numerous review and original artieies on specifictopies. We are also pleased - and you can attribute this to my own vanity - with the success and acceptance of the concept of myocard…ial reperfusion and revascularization - specifically selective intracoronary lysis followed either by PTCA or bypass surgery - which we have been pursuing since 1979. But after the dramatic immediate effects of the first attempts, which you have to experience yourself, we did not expect it to be any different. We decided against performing a randomized study in which every patient is catheterized, and thrombolytic therapy only given to some of the patients, for which we have been criticized at times. At the previous symposium in Aachen in 1983 on the topic of thrombolysis for acute myocardial infarction, in the final session several speakers were asked directly about this topic, and replied that if they were patients they would not want to be randomized into the placebo group. In the meantime positive results from large, randomized studies have been recorded, which are presented in this volume. I mention in particular the Western Washington Study from Dr. Kennedy and the Interuniversity Study in Holland. On the other hand, the recent advances in experimental cardiology emphasize the extremely complex and dynamie scenario of ischemic heart disease, involving endothelial damage, coagulation processes, metabolie and morphologie derangements, coronary constrictor mechanisms, blood flow redistribution, arrhythmogenesis, contraetile dysfunction during ischemia and reperfusion, and finally lack or presence of pain perception. Therefore, it appears desirable to close the gap between experimental and clinical cardiology and, thus, to provide a pathophysiologieal basis for rational cliniealdecisions with respect to diagnostic and therapeutic procedures. It may not be fully reflected in this collection of papers that the atmosphere of this meeting was unique. The reason for this resided apparently in the still rudimentary experiences with the adult cardiocyte model that is now widely acknowledged to be very promising. Thus, quite basic inter ests brought together experts from different biological disciplines who in respect to this new mod el were all still 'beginners'. It was noteworthy that such common interests exerted a unifying effect upon otherwise rather separated fields of science. However, the complex interplay b…etween the sympathetic initiation of myocardial ischemia, ischemia-induced alterations in sympathetic neurotransmission, as well as changes in adrenoceptor density and post-receptor signal transduction that can occur during ischemia remains incompletely understood. Not only the activation of myocardial fJ adrenoceptors, but also the activation of coronary IX-adrenoceptors can contribute to myocar dial ischemia. However, the role of fJ-adrenoceptor-mediated increases in contractility relative to heart rate in the initiation of ischemia is not clear, and the significance of IX-adrenoceptor mediated changes in coronary vasomotor tone, as well as the responsible IX-adrenoceptor subtypes are highly controversial. Malignant arrhythmias may be triggered by both IX- and fJ-adrenergic mechanisms. Current research in this field is focussed not only on the underlying physiological and pathophysiological mechanisms, but also on clinical treatment strategies, e. g. , by fJ-blockade, IX-blockade, bradycardic agents and calcium antagonists. Recent findings were presented and future research directions discussed during the 61' International Titisee Conference, held at the Schwarzwald-Hotel, Titisee, March 29-31, 1990 under the sponsorship of the Boehringer Ingelheim Foundation. Only 20 years ago, open-heart operations required an average of 8 units of blood preserves. The excessive need of donor blood in those early days was mainly due to premature surgical technique, insecure control of anticoagulation, severe blood trauma by extracorporeal circulation, and the lack of retransfusion technologies that would have allowed the reuse of shed mediastinal blood. The introduction of new technologies, such as normovolemic hemodilution, in traoperative autotransfusion, postoperative return of shed mediastinal blood, and predonation of autologous blood has greatly reduced donor blood requirements. At present the majority of routine coronary artery surgical procedures can be performed without any blood transfusion. Blood loss, however, may be considerable in patients undergoing complex valve surgery or reoperations, as they often require several units of transfused blood. Blood conservation has now become an area of major interest for the cardiac surgeon. In coronary heart disease the prognosis for patients is largely determined by the occurrence of irreversible ischemic damage to the myocardium. Irreversible eveniS are certainly preceded by a destabilization of a former stable situation. This desta bilization may occur with increasing symptomatology or without apparent warning signs to patient and physician; it may occur over a longer time period or very sud denly. The statemenl, however, holds true that no patient has ever died from stable angina pectoris. Thus, if the prognosis for patients with ischemic heart disease is to be improved, the focus has to be set on recognition and treatment of unstable ischemic syndromes. It was the goal of the conference to gather scientists and clinicans from different fields of research, to get an up-to-date view of the different aspects of unstable ischemic syndromes and to foster interdisciplinary discussions.

If asymptomatic ischemia did not carry prognostic significance comparable to the symp tomatic manifestations of ischemia, then the problem of silent ischemia would be very academic and without consequences for treatment. The following decade thus witnessed a number of such mostly unilateral lung transplants in several centres, in Germany represented by the two only lung transplants performed by E. S. Bücherl, then at the Neukölln City Hospital in Berlin in 1969. As with most other such attempts these two patients suffered early and lethai graft failure. There was only one single lung transplant patient who lived up to ten months after the transplant at Gent, Belgium, having been operated on by Derom in 1969. The alm ost universal failure during this initial phase was attributed to bronchial anasto motic insufficiency, pulmonary infection of either the transplanted lung or the left-in-place contralateral lung and a far-reaching lack of knowledge how to cope with transplant rejec tion.
