Seit 1894 a.o.Prof an der Univ. Kiel, ging er 1899 als o.Prof. an die Univ. Greifswald, 19…03 nach Bonn, lehrte 1907-32 an der Univ. Berlin und war dort Leiter der Chirurgischen Klinik. Bier, einer der führenden Chirurgen seiner Zeit, machte sich um die Behandlung von Amputationsstümpfen verdient, führte die Behandlung mit künstlicher Hyperämie (Hyperämie als Heilmittel") sowie die Rückenmarkanästhesie (Biersche Lumbalanästhesie") ein und setzte sich für die Homöopathie ein. 1920 wurde er Gründungsrektor der Deutschen Hochschule für Leibesübungen. Zu seinen Veröffentlichungen gehören u.a. Harmonisches System der Heilkunde" (1929), Neuere Gesichtspunkte in der Vererbung" (1938), Die Seele" (1939), Homöopathie und harmonische Ordnung der Heilkunde" (1939) und Das Leben" (postum 1951).
