Le thème principal du roman est la séparation , développé au centre d'une histoire, qui se veut réaliste, aussi bien dans son décor, ses péripéties, la description clinique de la maladie que la variété des personnages qui la composent. Elle narre comment la peste se déclare non dans une cité imaginaire, mais à Oran, et comment cette ville sera alors coupée du monde et livrée à son malheur. Cependant, quelques hommes sauront, par leur révolte, opposer au mal la seule attitude possible. La Peste a été le fruit d'une lente maturation : des notes éparses, des lectures, des idées qui germent et, un jour, le romancier sait qu'il tient un sujet et se met au travail. Cela a peut-être commencé par la lecture de la célèbre conférence d'Antonin Artaud Le Théâtre et la Peste, prononcée en 1933 à la Sorbonne, et publiée en 1934 dans La Nouvelle Revue Française (reprise dans le Théâtre et son double). Pendant les années 1941 et 1942, une grande épidémie de typhus fait des ravages en Algérie et, par un hasard étrange, fournit à l'auteur un modèle bien réel. Les victimes sont nombreuses dans les villages et dans les quartiers musulmans. Des zones entières sont interdites, frappés de quarantaine, comme Oran dans le roman. ''Je veux exprimer au moyen de la peste l'étouffement dont nous avons souffert et l'atmosphère de menace et d'exil dans laquelle nous avons vécu. Je veux du même coup étendre cette interprétation à la notion d'existence en général''. (In Carnets). La peste, c'est-à-dire la terreur de la souffrance et de la mort, l'enfermement, l'exil, même s'il s'agit de l'exil chez soi, la séparation, tel est le lot des hommes. Ils peuvent s'y abandonner, s'avouer vaincus, y voir la main d'un dieu châtiant on ne sait quel péché, ou bien retrouver leur dignité et leur liberté par la révolte, et la solidarité.''
