By definition, laryngeal cancer and appropriate laryngeal cancer surgery exact a considerable price from the basic human function of voice. By strict definition, voice is not a vital function - unlike a pumping heart, a respiring lung, or a functioning liver. Yet our ability to engage in verbal communication is a distinctly human characteristic and critical to our day-to-day interactions. Therefore, all efforts to surgically treat cancer of the larynx are simultaneously paralleled by efforts to afford acceptable voice rehabilitation. This concept dates back to the first total laryngectomy performed by Billroth in the 1870s and the subsequent efforts of Gussenbauer to restore voice. This issue of the Otolaryngologic Clinics of North America provides an overview of both of these therapeutic efforts. Each section addresses specifically how particular surgical procedures for treating laryngeal cancer are completed and how speech rehabilitation efforts can be directed specifically to the effects of such surgical intervention.
