La perfusion de chimiothérapie dans le foie isolé de la circulation générale permettrait de délivrer de f…ortes doses de drogue en l absence de toxicité systémique. Le cisplatine a montré une meilleure pénétration intracellulaire et une forte activité en milieu hypotonique. Cette étude expérimentale sur 10 porcs a consisté en une perfusion de cisplatine hypotonique sur foie isolé, grâce à un circuit ouvert entrant par la veine porte et sortant par la veine cave inférieure. En 30 minutes, 3 L de cisplatine en milieu hypotonique ont été perfusés. La tolérance per-opératoire a été bonne grâce à un clampage aortique intermittent. Les quantités de cisplatine dosées dans la circulation générale étaient minimes, confirmant l efficacité de l exclusion. Il n y a eu aucune toxicité systémique. La toxicité histologique hépatique existait aux concentrations de 100 et 75 mg/L, mais pas à 50 mg/L. L exposition in vitro à des cellules cancéreuses humaines hyper-résistantes a montré une cytotoxicité de 90%, signe d une forte activité antitumorale in vitro. 80 pp. Französisch.
