Auf Ersuchen Robert Kochs wurde er 1896 als Direktor an das Städtische Gesundheitsamt von Alexandria (Ägypten) gesandt; er hatte dieses Amt bis Kriegsbeginn 1914 inne. 1917 übernahm er das Ordinariat für Hygiene in Gießen, von 1926 bis zur Emeritierung 1935 das in Heidelberg. Bis 1941 leitete er das Zentral-Hygieneinstitut in Ankara (Türkei) und vertrat nach Kriegsende seinen alten Lehrstuhl in Heidelberg. Mit seiner Auffassung der Hygiene als Wissenschaft von den Umgebungsfaktoren unter Einbeziehung aller zivilisatorischen wie kulturellen Aspekte bekämpfte er in Ägypten 1899 die Pest und 1902 die Cholera. Emil Gotschlich (1870-1949 in Heidelberg) wissenschaftliche Tätigkeit umfaßte vor allem die Epidemiologie und die praktische Hygiene. Er forschte auf dem Gebiet der Infektionskrankheiten wie Pest, Cholera, Fleckfieber und Maltafieber und mit den speziellen Herausforderungen des Tropenklimas. Ihm gelang der Nachweis, dass der Pesterreger Yersinia pestis nach Ausheilung der Krankheit im Sputum noch wochenlang virulent ist. Seine Arbeiten gaben entscheidenden Aufschluss über die Rolle der Ratte bei der Verbreitung der Pest. Ferner forschte er zur Prophylaxe der Infektionskrankheiten sowie zu bakteriologischen Fragen der Desinfektion.
