"Clinical experiences and experimental studies provide evidence that an abnormal communication between t…he arterial and venous systems introduces into the circulation two circuits of flowing blood, one characterized by high arterial pressure and high peripheral resistance, and a second system bypassing part of the first, thus introducing a shorter circuit back to the heart, characterized by low pressure and low resistance, each supplied with flowing blood from the same central source, the heart. This shorter circuit acts solely as a parasitic circuit engrafted upon the normal circulation, serving no useful purpose, but capable of producing serious deleterious effects, depending upon the volume of blood diverted from the normal circulation into the shorter or parasitic circuit. Intimately dependent upon this diversion of differing volumes of blood into the shorter circuit is the great diversity in the effects of a peripheral fistula upon the heart: the complete absence of cardiac dilatation in some cases; the rapid development of cardiac dilatation and cardiac failure within 21/2 months as exemplified in Mason's case; and the long delay of 57 years in the development of cardiac failure as observed by Dorney. Due to the high pressure in the normal circuit and the low resistance in the fistula circuit, there is a constant tendency for blood to be diverted or sequestered, as it were, in the fistulous or parasitic circuit, the factors determining the volume of blood so diverted being the size of the fistula and the distensibility of its rim or border, this, in turn, being dependent upon the character of the fibrous tissue deposited around the fistula in the course of healing. Firm, rigid, and excessive scarring permits only a small volume of blood to be sequestered in the fistula circuit, dilating only mildly the components of the parasitic circuit, whereas an elastic and distensible fistulous border permits progressive expansion of the fistulous opening with a progressive increase in the volume of blood sequestered in the fistula circuit, causing progressive dilatation of all components of this circuit including the four chambers of the heart, the aorta and artery proximal to the fistula, the fistula itself, the proximal vein, the vena cava, and the pulmonary vascular bed. All gradations in distensibility of the fistula may occur, and hence all gradations in cardiac dilatation will occur. Also, all gradations in the rapidity of fistulous expansion may occur and, therefore, all gradations in rapidity of cardiac dilatation will occur. This concept of a progressive expansion of a fistula of slow or rapid development permitting more and more blood to be shunted or sequestered in a shorter parasitic circuit at very rapid or very slow rates of increase provides the long-sought explanation of the rapid or long-delayed development of cardiac dilatation and failure in the presence of a peripheral fistula. It is suggested that the same explanation is applicable to the effects of all abnormal arteriovenous communications, including intracardiac shunts and even some hemangiomata, since all are subject to the same hydrodynamic law that flowing blood like flowing water seeks the path of least resistance." "My dear Leah:/ I shall be greatly interested to note the result of the operations/ performed on you at the Johns Hopkins Hospital on August 8, 191…9, and/ August 26, 1919, by Dr. Holmes [sic]/ Will it be convenient to you to meet me in my office on the 4th floor/ of the Surgical Building, Johns Hopkins Hospital, on Friday morning,/ March 24, at 10.30 o'clock?/ Very truly yours,/ W. S. Halsted [signed], Surgeon-in-Chief." Halsted's resident Emile Holman wrote on the letter in red ink: "No recurrence No discomfort/ Incision well healed No hernia/ on left. Holman." Pencil note: "Information entered/ on cards/ MMR." Although the typed letter says the name of the surgeon was "Holmes", I feel sure it was meant to be "Holman". The surgeon who performed the operation was Dr. Emile Holman. Osler encouraged Holman to apply to medical school at Johns Hopkins, saying in his letter of recommendation for Holman, 'He is an A1 student and will be a great credit to the school. I do not think there is any doubt that he will have done enough work to warrant admission to the fourth year.' Holman was, in due course, accepted to Johns Hopkins Medical School in 1917 and graduated AOA [Alpha Omega Alpha] a year later. By this time, Holman had changed the spelling of his last name from a more Germanic Hollmann to Holman. After graduation from medical school, Holman worked in the Hunterian laboratories under Halsted and served as Halsted's last resident in 1922, the year of Halsted's death (James B. D. "IT MAY be categorically stated that a ductus arteriosus persistently patent after the fifth year of life, with or without symptoms, should be closed to avoid certain potential hazards. Should any of these complications appear before the age of 5, immediate operative closure of the ductus is indicated.1 One infant in our series was operated upon for cardiac failure at the age of 16 months, and we have just operated upon a second infant 5 months old in incipient cardiac failure with a pulse rate of 160 to 180, a respiratory rate of 80 to 100, and a blood pressure of 110/0. Another lesion may well be present but definite improvement has occurred as shown by a present blood pressure of 100/68. On the other hand, Josefson2 reported a patient who reached the age of 66 years before any harmful effects of the open ductus appeared. "In 1923, Lewis and Drury presented observations on arteriovenous fistulae in man and in the experimental animal which were quite limited in scope, in that the clinical fistulae were apparently not operated…upon, and the experiments dealt only with the immediate effects of a fistula in an animal which did not survive the experiment. Because of the obvious limitations of such observations, a number of the conclusions which were drawn up have since proved untenable. More specifically, cardiac output was said to be unchanged, whereas numerous observations indicate that it is markedly increased and even doubled in the presence of a fistula. Fistulae were said not to affect venous pressure, although some of their own observations, and many observations by others, show a very definite increase and sometimes doubling of venous pressure proximal to a fistula, depending on the size of the fistula. The dilatation of the heart was said to lie due to the effect of deficient nutrition incident to a lowering of mean arterial pressures, whereas Green showed an increased coronary flow due to systolic elevation, and others have demonstrated a degree of cardiac hypertrophy in dilated but properly functioning hearts totally incompatible with a deficient nourishment of cardiac muscle." Holman "Holman, in 1924, seems to have been the first to report an increase in blood volume in animals in which arteriovenous fistulas had been produced, with a return to normal level following the removal of these lesions. In his method the dye brilliant vital red was used to determine the blood volume. Four minutes after the dye was injected a sample of blood was drawn. The five animals in which he had produced particularly large arteriovenous communications all showed an increased blood volume, and in general the increase appeared to be related to the size of the fistula. Similar blood volume studies were reported later in clinical cases of arteriovenous fistulas, the increased blood volume in most instances returning to normal after correction of the lesions." "Cardiac hypertrophy and proximal dilatation of the artery have been so frequently assoc…iated with arteriovenous fistulas that it has long seemed highly probable that there must be some causative relationship between the two. That such a relation exists has been denied by clinicians, both ancient and modern, and the association of the two conditions clinically has been attributed to a coincidence. It was a problem that aroused the interest of the late Professor Halsted many years ago, and was for him a subject of constant speculation. Speaking before the American Surgical Asso-tion1 in 1918, he said: "If the assumption is correct that the heart dilates in consequence of the fistula, it is important that the fact should be brought to the attention not only of surgeons but also of pathologists and internists who apparently have altogether overlooked it . . . When a causative relationship . . . shall have become convincingly established, we may find that some unexplained dilations of the heart are referable to hitherto undetected changes in the walls and lumen of the blood vessels." Holman "Holman was the first to bring the Halstedian tradition to the West Coast. He established not only a surgical residency program but also the Halsted Laboratory of Surgical Research, in which he and a number of his faculty carried out important experiments. Holman is best known for his work on arteriovenous fistulas. His initial publication on this subject was in 1923, and his final one was in 1971. In between, he wrote more than 40 papers reflecting both laboratory and clinical observations and experimentation in this field. His original monograph, entitled "Arteriovenous Aneurysms: Abnormal Communications Between the Arterial and Venous," published in Circulation in 1937, had been awarded the coveted Samuel Gross prize of the Philadelphia Academy of Surgery in 1930.
