Une caractéristique importante de ces médicaments est leur localisation et leur rétention très sélectives dans l'os par rapport aux autres tissus. Les bisphosphonates agissent en réduisant le renouvellement des os, en augmentant la masse osseuse et en diminuant le risque de fracture. Ces dernières années, on a constaté qu'un petit sous-ensemble de patients (1-10%) traités par Bisphosphonates intraveineux présentent un risque accru de développer une ostéonécrose de la mâchoire (BPONJ). L'ostéonécrose est définie comme une zone osseuse exposée dans la région maxillo-faciale qui ne guérit pas dans les 8 semaines chez un patient qui reçoit actuellement des bisphosphonates et qui n'a pas été irradié dans la région tête-cou. La publicité faite autour de cette complication et le manque d'informations sur sa pathogenèse sous-jacente ont suscité une certaine appréhension chez les patients et les cliniciens dentaires lorsqu'ils traitent des patients ayant des antécédents de traitement aux bisphosphonates.
