In light of the above, basic research is a prerequisite for both diagnostic and thera…peutic advances. Thus it would seem justified to review the pathogenesis of hemoblastoses and the experiences resulting from those animal experiments which are trans ferable to human conditions. The association of Epstein-Barr virus infection with Burkitt's lymphoma appeared to be a model for the viral etiology of human neoplasias, the subject of many decades discussion. Although there is evidence for the correlation, the exact role of the virus in the etiology of the disease still remains to be clarified. In public, attention has been focused on the induc tion of neoplasia by environmental factors. For the hema tologist the induction ofhemoblastoses by immunosuppressive and cytostatic drugs gains increasing significance as a result of the wider use of these agents - not only cytotoxic cancer therapy, but also in treating autoimmune diseases and in managing transplantation problems. Physiology of the human stem cell has been intensively studied, and the presently available in vitro tests are of clinical use and enable greater understanding of pathophysiology, especially that of aplastic anemias and leukemias. Immunologic and biochemical mark ers have been of value in leukemias and malignant lym phomas, both as diagnostic tools and as prognostic parameters.

An…international symposium held in Innsbruck in 1978 was dedicated to clinical and pathophysiologic aspects of the preleukemic syndromes. This monograph consists of articles that correspond to the papers presented at the symposium and of the edited discussions. It was the aim of the symposium and of this monograph to outline comprehensively the pathophysiologic background of the preleukemic syndromes and to summarize the present information about their clinical management. The first part of the monograph covers that pathology of these disorders: Disturbed maturation and proliferation of the hemopoietic cells are the causes of cyto penias, of structural and ultrastructural anomalies, and of enzymatic and functional defects. Further articles are concerned with the evaluation of these features and of chromosome anomalies as possible tools for the diagnostic and prognostic classification of myelodysplasias. Zum Zeitpunkt der Diagnose hat das Bronchialkarzin…om bei 70% der Patienten bereits Metastasen abgesiedelt, bei 22% in die regionalen Lymphknoten und bei 48% hamatogen. Das erkHirt, warum viele Patienten innerhalb des ersten Jahres nach Diagnosestellung versterben. Aber auch bei den Kranken mit einer klinisch lokalisierten Erkran kung (17%) ist eine Oberlebenserwartunvon 5 Jahren eher die Ausnahme als die Regel. Viele Hoffnungen wurden bei diesen Gegebenheiten auf die Friihdiagnose ge setzt, urn den Tumor im loko-regional begrenzten Stadium der Operation zu fUhren zu konnen, die fast allein als kurative BehandlungsmaBnahme anzuse hen ist. Die Friihdiagnostik hat jedoch die in sie gesetzten Erwartungen nicht erftillen konnen. Feldstudien in den USA und der DDR haben eine Verbesse rung der Oberlebensraten nicht erreichen konnen. Die Moglichkeiten der Friihdiagnose sind weiterhin beschrankt, Vorsorgeuntersuchungen fUr die Ge samtbevolken,mg sind deshalb nicht empfehlenswert. Die folgenden Beitrage namhafter Forscher und Kliniker sollen den Stand des heute Moglichen in der Behandlung von Kranken mit Lungenkrebs diskutieren und Wege zur Verbesserung der Ergebnisse aufzeigen. Auch von den in kurati ver Absicht radikal Operierten leben nach 5 Jahren nur rund 30%, nach 10 Jah ren 15%. Die Operationsletalitat betragt je nach der GroBe des Eingriffs 4-11 %.
