Akuter rheumatischer Rheumatismus tritt häufig im Alter von 10 bis 12 Jahren auf und erfordert die sofortige Beseitigung von Streptokokken sowie eine anschließende Dauer…behandlung mit Benzathin-Benzylpenicillin oder Sulfonamiden. Im akuten Stadium wird Prednisolon für 4 bis 6 Wochen verordnet, ergänzt durch Acetylsalicylsäure. Bei progressiv-chronischem Rheumatismus, der häufig bei Erwachsenen zwischen 40 und 60 Jahren auftritt, ist eine Kombination aus medikamentöser Therapie und effektiver Krankengymnastik entscheidend. Chemotherapie und Herdsanierung sind nicht angezeigt. Die medikamentöse Dauerbehandlung kann Acetylsalicylsäure, Phenylbutazon oder Indometacin umfassen. Prednisolon wird nur bei akuten Verschlimmerungen eingesetzt und sollte nicht als Ersatz für Analgetika verwendet werden. Bei Bedarf kann eine Basistherapie mit D-Penicillamin in Betracht gezogen werden, wobei eine sorgfältige Überwachung notwendig ist, um gefährliche Nebenwirkungen zu vermeiden. Die Erfolgsquote liegt bei 30% für vollständige Remission. Bei Spondylarthritis ankylopoetica, die häufig erblich bedingt ist und Männer stärker betrifft, sind regelmäßige Bewegungsübungen wichtig. Die Dauerbehandlung erfolgt hauptsächlich mit Indometacin, das als besonders wirksam gilt.
