Wien und ging 1879 zur weiteren Ausbildung u.a. zu Max von Pettenkofer nach München. 1882 habilitierte er sich an der Univ. Wien für Hygiene, arbeitete bei dem Physiologen Carl Ludwig in Leipzig und wurde 1884 a.o. Prof. der Hygiene in Graz, 1887 in Wien, 1891 o. Prof. und übernahm die Leitung des Hygienischen Instituts. Von 1902 bis zu seiner Emeritierung 1923 lehrte er als o. Prof. der Hygiene in München. Gruber, der 1908 persönlich geadelt wurde, hatte maßgebend Anteil an der Entwicklung der modernen Hygiene, war an der Sanitätsgesetzgebung Österreichs und Bayerns beteiligt und beschäftigte sich mit Fragen der Jugenderziehung, Schulhygiene, Alkoholismus, Prostitution und Städtesanierung. Auf dem Gebiet der Bakteriologie und Immunitätsforschung entdeckte er die spezifische Agglutination der Typhusbakterien durch das Typhusserum (zusammen mit Durham 1896 veröffentlicht). Er gehörte zu den Herausgebern des Handbuchs der Hygiene (1911ff.) und war Mitglied der Bayerischen Akademie der Wissenschaften (1924 ihr Präsident) sowie der Österreichischen Akademie der Wissenschaften.
