Die benotigten groBen Initialdosen haben jedoch unkontrollier bare Nebenwirkungen auf die Atmung und den Kreislauf. Wahrend wir heute die Atemdepression durch eine sofort einsetzende assistierte Beatmung kompensieren konnen, sind die negativen Wirkungen auf das kardiovaskulare System auch bei vorsichtiger Dosierung in ihrem AusmaB schlecht vorherzusehen und bedeuten ein betrachtliches Risiko fUr den Patienten. Dagegen ist die Einleitung der Narkose mit einem Inhalationsanaesthetikum ein schonenderes Verfahren, das jedoch wegen der verlangerten Ein leitungsphase (202) und der haufigen Ablehnung durch den Patienten in der taglichen Praxis wenig angewendet wird. Das Herz-Kreislaufversagen nach intravenoser Narkoseeinleitung ist auf eine Abnahme der Myokardkontraktilitat und/oder auf einen Abfall des peripheren Gesamtwiderstandes zurUckzufUhren Ci, 144, 211, 212). Sowohl eine verringerte Pumpleistung des Herzens als auch ein ver minderter venoser RUckstrom als Folge einer peripheren Vasodilatation bewirken einen Abfall des Herzzeitvolumens und des Blutdruckes und konnen damit zu einer Minderperfusion der Organe fUhren. Da die ver anderte hamodynamische Situation eine akute Anpassung der Sauerstoff versorgung des Herzens erfordert, kann es darUber hinaus bei Patienten mit eingeschrankter kardialer Leistungsbreite, wie Koronarsklerose, Hypertonus. Herzinsuffizienz. Herzklappenerkrankungen und im Schock syndrom zu einer negativen Sauerstoffbilanz des Herzens kommen, die eine Storung des myokardialen Stoffwechsels bewirkt und die Myokard depression derart verstarkt, daB ein Kreislaufstillstand resultieren kann.
