Lymphatic filariasis, a neglected tropical disease, remains a significant cause of chronic lymphoedema and long-term disability in many endemic regions, including Sri Lanka. Although the country has achieved elimination of filariasis as a public health problem, new cases of lymphoedema continue to emerge each year, highlighting the ongoing need for effective morbidity management. This study examines the management practices of lymphoedema patients attending specialized clinics in selected endemic districts of Sri Lanka, including Kalutara, Puttalam, and Gampaha. Using a clinic-based analytical cross-sectional design, data were collected from 405 patients to assess their knowledge, attitudes, and practices related to lymphoedema care, as well as the socio-demographic and clinical factors influencing these practices. The findings reveal that while patients generally demonstrate good knowledge and positive attitudes towards disease management, significant gaps remain in the consistent application of recommended self-care practices such as exercise, limb elevation, and compression. Factors such as education level, income, and quality of life were found to significantly influence management behaviors. This work provides valuable insights into the challenges of sustaining effective lymphoedema management in post-elimination settings. It highlights the need for strengthened patient education, improved health system support, and targeted interventions to enhance self-care practices and reduce disease burden.
