In this review of home care, we will exclude normally ambulatory persons who use health care technology at home, like devices to self-manage illnesses such as diabetes, asthma, and hypertension, and tools to interact with health care providers. We will include people with functional limitations who are normally mobile and independent because of adaptive technology, such as otherwise healthy persons with paraplegia equipped with appropriate wheelchairs, ramps, elevators, and vans. These people require costly devices and supplies, plus home-health services when complications ensue, but not ongoing supportive care or case management. On the far end of the morbidity spectrum are those who are bedfast and completely dependent. Intermixed are persons of all ages with degrees of chronic illness burden and an enormous variety of conditions, short- and long-term care needs, upward and declining trajectories, those who are recovering and those who are dying. They share a common bond: the central locus of care is their home.
