Auflage und neben dem umfassenden Klinischen Wörterbuch - hat verschiedene Gründe. Zum einen ist Willibald Pschyrembel, der das Klinische Wörterbuch zur Blüte gebracht hat, selbst Frauenarzt gewesen. Zum anderen ist die Frauenheilkunde - vielleicht wie kein anderes klinisches Fach - ein exemplarisches Stück Ganzheitsmedizin. Dieser Anspruch bezieht sich nicht nur auf die Frau als ?Betreuungs- und Stud ienobj ekt", sondern erstreckt sich auch auf den Mann, soweit es sich um sterile Paare handelt. Daher enthält das Wörterbuch auch Begriffe aus der Andrologie, deren Bezug zur Gynäkologie sich aus dieser Betrachtungsweise ableitet. Zum dritten wurden und werden dje Potenz, der Ideenreichtum und die Visionen der Frauenheilkunde aus vielen Quellen gespeist - nicht zuletzt auch ein Indiz für besondere Integrationsfähigkeit. Freilich hat unser Fach im Gegenzug auch Meilensteine gesetzt, die andere Disziplinen nachhaltig bereichert haben. Erwähnt seien in diesem Zusammenhang stellvertretend die Endokrinologie, die Ultraschalldiagnostik und die minimal-invasive Chirurgie. Selbst die theoretische Medizin - unsere naturwissenschaftliche Grundlage also - wurde und wird durch die Frauenheilkunde spürbar beeinflußt. Als Beispiele hierfür seien die jüngsten fundamentalen Erkenntnisse auf den Gebieten der Reproduktionsvorgänge (bis hin zur Klonierung von Individuen) sowie der Onkologie (etwa Gendiagnostik und-therapie von Malignomen) genannt. Auch aus der Sicht der täglichen Praxis kann man sagen, daß in keinem anderen Fach der Bogen vom Beginn bis zum Ende des Lebens in dieser einzigartigen Weise geschlagen wird. Es verwundert daher nicht, daß diejanusköpfigkeit der modernen Medizin gerade in der Frauenheilkunde ihren besonderen Ausdruck findet. Das ethisch-moralische Spannungsfeld unseres Faches läßt sich auch mi t Begriffen wie Kontrazeption, intrazytoplasmatische Spermieni njektion, Präimplantationsdiagnostik, K lonierung, Fetozid und Schwangerschaftsabbruch umreißen. Wie in keiner anderen medizinischen Disziplin werden hier Zwänge der gesellschaftlichen Erwartungen, des ethisch Vertretbaren sowie des technisch und ökonomisch Machbaren deutlich; Mode* begriffe wie das Kind ?nach Maß", das Kind ?als Schaden" oder der Frauenarzt als ?williger Vollstrecker" der Gesellschaft, sind gängige und vom Zeitgeist geprägte Vokabeln. Hinzu kommt, insbesondere in der Geburtshilfe, ein immenser und noch zunehmender forensischer Druck. Eine heute schon aktuell vorstellbare Situation, die den Frauenarzt quasi zum ?Herrn über Leben und Tod" macht, muß uns zu tiefgründigen Überlegungen und Einsichten über die Sinnhaftigkeit der modernen Heilkunde führen. Ein weiteres Spezifikum unseres Faches: in der Reproduktions- und Geburtsmedizin haben wir es mit zwei Leben zu tun. Aus dieser Konstellation leitet sich die besondere Verantwortung des Frauenarztes ab, die auch die Verantwortung zur kontinuierlichen Fortbildung einschließt. Das hier vorliegende Wörterbuch will diesbezüglich einen Beitrag leisten. Das Fach Gynäkologie und Geburtshilfe kann von einem einzelnen nicht mehr wie vielleicht noch von Willibald Pschyrembel - enzyklopädisch übersehen und erfaßt werden. Daher wollen die Stimmen, die einer Teilung unseres Faches das Wort reden, nicht verstummen. Die neue Weiterbildungsordnung mit den drei fakultativen Weiterbildungen folgt - auch unter dem Diktat der Qualitätssicherung - europäischen Vorstellungen und könnte ein erster, wenn auch noch unentschlossener Schritt in diese Richtung sein. Eine andere ?klassische" Form der Versel bständigung, nämlich die Trennung in Gynäkologie und Geburtshilfe als eigenständige Fächer, ist in mancher deutschen Frauenklinik bereits problematische Wirklichkeit. Daneben werden Abspaltungstendenzen erkennbar, wie etwa die Übernahme von Mammaerkrankungen durch die plastische Chirurgie, der Osteoporose durch die Orthopädie, des Ultraschalls durch die Radiologie, der Chemotherapie durch die internistische Onkologie, der Mädchen unter 18 Jahre durch die Kinderheilkunde, der Geburtshilfe durch Hebammen und anderes mehr. Ziel dieses Wörterbuches ist daher auch, ausufernden und nicht konsequent zu Ende gedachten Spaltungsund Abspaltungstendenzen entgegenzuwirken. Das Pschyrembel Wörterbuch Gynäkologie und Geburtshilfe ist nicht einfach eine Auskoppelung aus dem Klinischen Wörterbuch. Manches wurde übernommen, manches auf den neuesten Stand gebracht; vieles - sowohl Texte als auch Abbildungen - kam neu hinzu. Das Wörterbuch enthält 4221 Stichwörter mit 348 Abbildungen, 82 Tabellen und 26 Kastentexten.
