The aim of our study was to evaluate the effect of low-dose corticosteroid therapy on the incidence of pulmonary embolism (PE) in polytrauma patients. Mater…ial and methods: We conducted a comparative study of two cohorts: a retrospective cohort without corticosteroid therapy (C-) and a prospective cohort with hydrocortisone hemisuccinate corticosteroid therapy (HSHC) at a dose of 300 mg per day for 7 days (C+).Results:175 polytrauma patients were included (83 patients for the retrospective cohort and 92 patients for the prospective cohort). PE was diagnosed in 15 patients (8.5%). The incidence of PE was significantly lower in the groups receiving corticosteroids (p=0.013). There was no significant difference between the C(+) and C(-) groups in terms of length of ICU stay (p=0.372) or ICU mortality (p=0.434).Conclusion: Low-dose corticosteroid therapy significantly reduces the incidence of symptomatic PE in polytrauma patients, without affecting ICU mortality.VDM Verlag, Dudweiler Landstraße 99, 66123 Saarbrücken 68 pp. Englisch.

Material y métodos: Se realizó un estudio comparativo de dos cohortes: una cohorte retrospectiva sin tratamiento corticoideo (C-) y una cohorte prospectiva con tratamiento corticoideo a base de hemisuccinato de hidrocortisona (HSHC) a dosis de 300 mg al día durante 7 días (C+).Resultados:Se incluyeron 175 pacientes politraumatizados (83 pacientes en la cohorte retrospectiva y 92 pacientes en la cohorte prospectiva). Se diagnosticó EP en 15 pacientes (8,5%). La incidencia de EP fue significativamente menor en los grupos que recibieron corticosteroides (p=0,013). No hubo diferencias significativas entre los grupos C(+) y C(-) en cuanto a la duración de la estancia en cuidados intensivos (p=0,372) o la mortalidad en cuidados intensivos (p=0,434).Conclusión: El tratamiento con dosis bajas de corticosteroides reduce significativamente la incidencia de EP sintomática en pacientes politraumatizados sin afectar a la mortalidad en la UCI.VDM Verlag, Dudweiler Landstraße 99, 66123 Saarbrücken 72 pp. Spanisch.

Material und Methoden: Wir führten eine vergleichende Studie mit zwei Kohorten durch: eine retrospektive Kohorte ohne Kortikosteroidtherapie (C-) und eine prospektive Kohorte mit einer Kortikosteroidtherapie mit Hydrocortisonhemisuccinat (HSHC) in einer Dosis von 300 mg pro Tag über 7 Tage (C+).Ergebnisse:175 Polytraumatisierte wurden eingeschlossen (83 Patienten in der retrospektiven Kohorte und 92 Patienten in der prospektiven Kohorte). Bei 15 Patienten (8,5%) wurde eine PE diagnostiziert. Die Inzidenz von PE war in den Gruppen, die von der Kortikosteroidtherapie profitierten, signifikant geringer (p=0,013). Es wurde kein signifikanter Unterschied zwischen den Gruppen C(+) und C(-) gefunden, weder hinsichtlich der Dauer des Aufenthalts auf der Intensivstation (p=0,372) noch hinsichtlich der Mortalität auf der Intensivstation (p=0,434).Schlussfolgerung: Eine niedrig dosierte Kortikosteroidtherapie führt zu einer signifikanten Verringerung der Inzidenz symptomatischer PE bei Polytraumatisierten, ohne die Mortalität auf der Intensivstation zu verändern. 72 pp. Deutsch.
