Laut verschiedenen Studien schwanken die Angaben zwischen 25 bis 75% bei Patienten von Allgemeinärzten (z. B. Katon, Ries & Kleinmann, 1984) und bei ca. 30% in Krankenhäusern (z.B. Kirmayer & Robbins, 1991). Die Betroffenen suchen - da sie der Überzeugung sind, eine organische Krankheit zu haben - häufig Ärzte und Krankenhäuser auf, was dazu führt, daß bei ihnen unnötige bzw. mehrfache Untersuchungen, falsche Diagnosen bis hin zu überflüssigen Operationen durchgeführt werden. Daraus ergeben sich neben enormen Kosten für das Gesundheitssystem auch - bei richtiger Behandlung vermeidbare - Folgeschäden für die Patienten.Neben den diagnostisch relevanten somatoformen Symptomen sind die Erkrankungen durch kognitive, affektive, verhaltensbezogene und soziale Aspekte gekennzeichnet. Ziel der Arbeit war es, Kognitionen bei Patienten mit somatoformen Störungen näher zu erforschen. Kognitionen sind bei anderen psychischen Störungen, z.B. Depressionen, Angst und Sucht schon gut erforscht. Es sollten dysfunktionale Einstellungen, Denkweisen und Bewertungen bei somatisierenden Patienten genauer untersucht werden, da inadäquate Kognitionen bei der Entwicklung und Aufrechterhaltung der verschiedenen psychischen Störungen - so auch des Somatisierungssyndroms - eine wichtige Rolle spielen und Kognitionen im Zuge somatoformer Störungen noch nicht so umfassend untersucht wurden wie beispielsweise bei den oben genannten psychischen Störungen.Zur Feststellung der dysfunktionalen Kognitionen wurde der Fragebogen zu Körper und Gesundheit (FKG) eingesetzt. Der Fragebogen wurde von insgesamt 85 zur stationären Behandlung aufgenommenen Patienten der medizinisch-psychosomatischen Klinik Roseneck ausgefüllt. Die 85 Patienten wurden mittels SSI-8 Kriterium in Experimental- und Kontrollgruppe aufgeteilt und bezüglich der fünf Skalen des FKG miteinander verglichen. Weiters wurden die mit dem Fragebogen zu Körper und Gesundheit (FKG) erhobenen Kognitionen mit anderen wichtigen Aspekten (Anzahl der Krankheitstage, Rente, Anzahl der Konsultationen bei Behandlern, Einschränkungen bei üblichen Alltagstätigkeiten, Bettlägrigkeit, Medikamenteneinnahme, Depression, Lebensqualität) somatoformer Störungen in Beziehung gesetzt.Screeningphase: Zu Beginn des stationären Aufenthalts wurde allen Patienten der ausgewählten Stationen der Fragebogen SOMS-2 (Screening für Somatoforme Störungen) vorgelegt. Die Versuchspersonen mußten mindestens 8 Symptome im SOMS-2 bejaht haben, um in die Untersuchungsstichprobe aufgenommen werden zu können.Erster Teil des Interviews: Mit den Patienten wurde das Interviewverfahren SDS (Somatoform Disorders Schedule) zur Diagnostik somatoformer Störungen und das Mini-Dips (Diagnostischen Interview Psychischer Störungen, Kurzform) zur Abklärung weiterer psychischer Störungen durchgeführt.Fragebogenbearbeitung durch die Patienten: Fragebogen zu Körper und Gesundheit (FKG), Beck Depressions Inventar (BDI).Zweiter Teil des Interviews: Dieser Interviewteil enthielt Interviewverfahren zu folgenden Bereichen: Lebensqualität, Problemlösestil, Kognitionen, Therapieziele.Die Daten wurden mit Hilfe des Statistikprogramms SPSS ausgewertet. Je nach Hypothese wurde entweder ein t-Test für unabhängige Stichproben verwendet oder eine Produkt-Moment-Korrelation nach Pearson berechnet.Gang der Untersuchung:Der theoretische Teil der Arbeit gliedert sich in 1. Ziel der Arbeit war es, Kognitionen bei Patienten mit somatoformen Störungen näher zu erforschen. Kognitionen sind bei anderen psychischen Störungen, z.B. Depressionen, Angst und Sucht schon gut erforscht. Es sollten dysfunktionale Einstellungen, Denkweisen und Bewertungen bei somatisierenden Patienten genauer untersucht werden, da inadäquate Kognitionen bei der Entwicklung und Aufrechterhaltung der verschiedenen psychischen Störungen - so auch des Somatisierungssyndroms - eine wichtige Rolle spielen und Kognitionen im Zuge somatoformer Störungen noch nicht so umfassend untersucht wurden wie beispielsweise bei den oben genannten psychischen Störungen.Zur Feststellung der dysfunktionalen Kognitionen wurde der Fragebogen zu Körper und Gesundheit (FKG) eingesetzt. Der Fragebogen wurde von insgesamt 85 zur stationären Behandlung aufgenommenen Patienten der medizinisch-psychosomatischen Klinik Roseneck ausgefüllt. Die 85 Patienten wurden mittels SSI-8 Kriterium in Experimental- und Kontrollgruppe aufgeteilt und bezüglich der fünf Skalen des FKG miteinander verglichen. Weiters wurden die mit dem Fragebogen zu Körper und Gesundheit (FKG) erhobenen Kognitionen mit anderen wichtigen Aspekten (Anzahl der Krankheitstage, Rente, Anzahl der Konsultationen bei Behandlern, Einschränkungen bei üblichen Alltagstätigkeiten, Bettlägrigkeit, Medikamenteneinnahme, Depression, Lebensqualität) somatoformer Störungen in Beziehung gesetzt.Screeningphase: Zu Beginn des stationären Aufenthalts wurde [¿]Diplomica Verlag, Hermannstal 119k, 22119 Hamburg 168 pp. Deutsch.
