"We have had in the neurologic service of the University of Pennsylvania a number of patients in whom paralysis of one lower limb was associated wit…h atrophy, exaggerated tendon reflexes of the affected limb and in some instances with spasticity. The first case of this kind was reported by Dr. Williams B. Cadwalader,1 June, 1916, and is given as Case 2 of this paper. Since that time a considerable number of cases have been under observation, but it was not possible to devote sufficient time to scientific work during or immediately following the recent war. This form of monoplegia has also been studied by 2 and in all his cases the upper limb was affected. He had no necropsies. He suggests the term monoplegia spinalis spastica superior. In all my cases the lower limb has been the one affected, so that a lower as well as an upper type occurs, a monoplegia spinalis spastica inferior. It seems preferable therefore to describe the condition simply as monoplegia spinalis spasctica, or in the English equivalent, spastic spinal monoplegia." Rare Offprint! "Up to the present time I have done eleven operations for the removal of the Gasserian ganglion. Th e first of these cases was reported in th…e Transactions of the Philadelphia County Medical Society for 1894 and in the Medical ainl Surgical Reporter for March 1894 in conjunction with Dr. John K. Mitchell. It was the fourteenth operation done on the patient. From the date of that operation, October 18th, 1893, the patient has been well, though his condition of mental anxiety lest the pain should return and his mental instability as a. result of his long continued use of opium are very marked, yet practically he is well of his neuralgia. He still has pain, but not the old tic. In the American Journal of the Medical Sciences for January 1896, I reported live additional cases. The later history of these cases is as follows :" W.W. Keen, Jefferson Medial College (Philadelphia) "Little attention has been paid to the microscopical examination of the Gasserian ganglia removed from patients suffering from tic douloureux. Indeed, with the exception of a tew cases, we have had almost no information furnished us on the pathological condition of these structures in the disease. It was, therefore, with a feeling of the deepest interest that I undertook the examination of the ganglia removed by Professor Keen from seven cases of prosopalgia. This report is based on a study of several hundred sections. The stains employed have been carmine, hematoxylin (Delafield's and Weigert's), osmic acid (methods of Azoulay and of Marchi), thionin, acid fuchsin and eosin. The method of Rosin has given most satisfactory results for a study of the nerve fibres, and in some cases, in which the material did not stain readily with carmine, Delafield's hematoxylin, with an after-stain of acid fuchsin, presented beautifully colored specimens. Unfortunately, the method of Nissl could not be employed, as all the ganglia had been placed in Muller's fluid. The method of Azoulay proved to be of great service, for in some cases in which the method of Weigert did not give thoroughly satisfactory results, the former yielded sections well stained." W.G. He initially performed peripheral nerve section, with the usual poor results. Later he began removing the entire ganglion, and had numerous successes. Keen stressed the need for lowering the morbidity and mortality of the operation, and described difficulties and complications such as hemorrhage and corneal ulceration. "FIRST SUCCESSFUL FM MENINGIOMA RESECTION…" "Since the first autopsy description of a foramen magnum (FM) meningioma was presented by Hallopeau in 1872, it has been appreciated that lesions in this region may present many challenges to a surgeon attempting resection. FM tumors are located in an anatomically complex region surrounded by bony elements of the craniocervical junction, the vertebrobasilar system of vessels, the lower cranial nerves, and by delicate brainstem neural elements. Since the first successful FM meningioma resection in 1922 was reported by Frazier and Spiller, a number of surgical refinements have been introduced to aid in the management of these technically demanding lesions. The use of microneurosurgical techniques with modern skull base approaches now allows for definitive surgical management with an acceptable risk profile in most cases. Expanded endoscopic endonasal approaches are also now being performed in attempt to further minimize surgical morbidity in removing these lesions. However, in those cases where definitive surgical management cannot be performed due to age, medical condition, or patient preference, radiosurgery is also a viable option and has been utilized with some success."
