In dieser Arbeit werden die vorrangig medizin-ethischen und wissenschaftstheoretischen Bedingungen untersucht, die erfüllt sein müssten, damit sich die Theorie einer Krebsfrüherkennung zum Wohl des Patienten umsetzen lässt: Wer ist kompetent, den jeweils stochastisch zu interpretierenden Nutzen und Schaden adäquat gegeneinander abzuwägen Was sind angemessene Evaluationsparameter Gibt es Grenzen der Informationsvermittlung in einer partnerschaftlichen Arzt-Patienten-Beziehung Vor dem Hintergrund der früherkennungsspezifischen Relativität von medizinischem Wissen und der unklaren Nutzenbestimmung stellt sich die Frage, wie verantwortungsvolles ärztliches Handeln aussehen kann. Der alleinige Hinweis auf die ärztliche Pflicht zur Aufklärung und Stärkung der Eigenverantwortung von Patienten verliert sich dabei zum Teil in paradoxen Scheinlösungen. Durch die genannten Probleme in der Krebsfrüherkennung wird deutlich, dass die Bedingungen für die Möglichkeit von patientenorientierter Informationsvermittlung nur schwer erfüllbar sind.

Die Befürworter bedienen sich vornehmlich eines pragmatisch-nutzenorientierten Argumentationstyps. Hierbei werden die Interessen und Bedenken möglicher Patienten aus einer subjektiven und situativen Perspektive heraus nach Pro- und Kontraargumenten gegeneinander abgewogen. Auf der anderen Seite folgen die Kritiker der Präimplantationsdiagnostik einem gesellschaftspolitisch-folgenorientierten Argumentationstyp. Sie weisen neben dem möglichen Schaden für die Patienten auf das Negativpotential dieser Technik hin. Dabei werden in den abwägenden Bewertungsprozess neben den technikspezifischen Aspekten weitere heterogene Diskussionsfelder integriert.Die vorliegende Arbeit gibt zunächst einen Überblick über die naturwissenschaftlichen, klinischen und rechtlichen Aspekte zur Präimplantationsdiagnostik. Anschließend wird der beschriebene Diskurs in seinen spezifischen Argumentationssträngen analysiert. Die ebenfalls beteiligten Diskussionsfelder zum humangenetischen Krankheitsbegriff, zur Patientenautonomie, zum Embryonenstatus und zur Eugenik-Debatte werden in Beziehung gesetzt zur Thematik der Präimplantationsdiagnostik.Ich stelle fest, dass die unterschiedlichen Ansätze der beiden wichtigsten Argumentationstypen eine grundlegende Asymmetrie bedingen. Aufgrund der hochgradigen Komplexität des Bewertungsprozesses ist der gesellschaftspolitische Argumentationstyp anders als der pragmatische mit einem latenten Vermittlungsproblem belastet.
