Édition originale rare de la thèse de l'auteur. J.F.B. Maurice Treille, né en 1782 à Bassoues (Gers), était chirurgien-major des armées ; il participa à la bataille de Bailén, en Andalousie, opposant l'armée napoléonienne à l'armée espagnole. Et cette thèse est basée en grande partie sur la propre expérience de l'auteur en Andalousie. Treille deviendra chirurgien-major des sapeurs-pompiers de la ville de Paris. Dans cette thèse, Treille explique qu'après la bataille de Baylen, il resta "sur le champ de bataille seul chirurgien pour y soigner cinq cents blessés". Ces patients demeurèrent couchés, pendant 3 semaines, "sous le ciel brûlant de l'Andalousie, n'ayant pour tout ombrage que les faibles rameaux de l'olivier". Treille expose la conduite qu'il a tenu : "Privé de tout médicament, j'arrosai toutes les plaies avec de l'eau pure. Je continuai mes pansements de cette façon pendant vingt-un jours que nous restâmes sur le champ de bataille, ne recevant que du linge et des aliments". Devant le grand nombre de blessés, certains malades se pansaient eux-mêmes. Sept à huit plaies seulement se gangrénèrent, et il n'y a eu que deux tétanos. Treille attribue le faible cas de ces complications au fait d'arroser les plaies avec de l'eau pure, rejoignant Larrey et Percy sur les effets salutaires de l'eau. Par ailleurs, Treille indique qu'à l'hôpital de Toulouse, il observa fréquemment, "dans les plaies gangrénées, sphacélées, une complication d'irritation ou de phlegmasie de la membrane muqueuse de l'estomac", avec, à l'ouverture du cadavre, "la membrane muqueuse de l'estomac enflammée, et quelquefois ulcérée". Il s'agit de la première description connue de l'ulcère de stress. In his care were five hundred severely wounded soldiers lying under the hot sky of Andalusia, without protection of hospital or cover except from olive trees. He had no bandages or medicaments so he irrigated their wounds with water to forestall pourriture. Because he could not treat that many patients daily unaided, he divided the men into three groups, alternately irrigating the wounds of one-third of the patients each day while the other two-third irrigated their own wounds. Over the three-week interval from June 19 to July 10, 1808, only seven or eight wounds became gangrenous and there were only two cases of tetanus. Treille believed the most effective agent in combatting pourriture d'hôpital was the actual cautery. In autopsies of patients succumbing from gangrenous wounds, Treille observed and reported the frequent presence of gastric erosions and superficial mucosal ulcers, probably the first mention of stress ulcers in surgical literature.
