In the early 20th century, the infant mortality rate was alarmingly high in many urban industrial centers in the United States, and a study was conducted in New Bedford, Massachusetts to investigate the causes of the excess mortality in that city. In 1913, infant mortality rates were unusually high in New Bedford and showed great variance among the different precinctsâ"some areas had infant mortality rates more than three times higher than others. The first comprehensive analysis of infant mortality in New Bedford revealed that many factors were at play: a large immigrant population composed of distinct nationalities and cultures, widespread employment of mothers in low-paying textile mills, inadequate health services, and unsanitary housing conditions in many of the poorer mill neighborhoods. The study found that foreign-born mothers had much higher infant mortality rates than native-born mothers, and that Portuguese mothers had the highest rates of all. Illiteracy and lack of English proficiency were associated with higher mortality rates, as these mothers were less likely to access educational materials and medical advice. In those days, improper infant feeding practices, such as not breastfeeding, were widespread, and in New Bedford, the situation was exacerbated by the large number of mothers employed in the millsâ"the percentage of infants who were breastfed in the first month of life was much lower in New Bedford than in the other cities studied by the author. But even among infants who were breastfed, mortality rates varied according to the nationality of the mother. Among the Portuguese, significantly fewer babies were breastfed, and those who were breastfed were more likely to receive mixed or artificial feedings. Ultimately, this study led to significant improvements in infant health in New Bedford.
