Through this preliminary study, the researchers shed light on the experiences of inmates in Indian prisons, with an aim of presenting questions for future research. The author provides an overview of the global conditions of prison inmates living with HIV, as an international comparative context for examining the cases in India. Major problems highlighted in the cases include: living conditions, high risk behavior during incarceration, delivery of medical services and adherence to ethical guidelines. Results of the study reveal that overcrowding and inadequate nutrition were major concerns for inmates living with HIV; there were no support systems available inside the prisons to address the stress related issues of the inmates; and, the prison hospital did not have provisions to cater to the treatment needs of inmates living with AIDS. The study also found that confidentiality regarding the HIV positive status could not be maintained inside the prison. This Brief presents a window into the experience of inmates in India, and presents questions for future research to understand and improve living conditions and medical service delivery within the prison system. Not only are the number of randomized controlled trials (RCTs) in Nephrology the third lowest amongst the medica…l specialties but in many instances the trials themselves are of poor quality. In addition, practice has been further influenced by extrapolation from the outcomes of general population clinical trials which exclude renal patients. The difference between the ideal trial participant and real complex cases encountered in daily practice is well recognized and further compounded in renal patients with complex pathophysiology - this ultimately makes decision making in this subset of patients a real challenge. Recently, there has been a growing interest in conducting well designed RCTs in different areas of renal medicine. However, though clinical guidelines are helpful in providing the clinicians with a frame of best available evidence for a clinicalcondition, it denies the unique nature of each individual patient.This book offers a thorough and critical appraisal and evaluation of the key published clinical trials that have shaped current practice in nephrology, dialysis and transplantation. It will help the practicing physician close the gap between the inflexible and generalized nature of clinical guidelines and the day-to-day clinical decision-making for individual patients. It is the result of a theoretical development and examination of the generalized adjoint concept and the conditions under which systems analysis using ad…joints is valid. Results developed in this book are useful aids for the analysis and modeling of physical systems, including the development of guidance and control algorithms and in developing simulations. The generalized adjoint system is defined and is patterned similarly to adjoints of bounded linear transformations. Next the elementary properties of the generalized adjoint system are derived. For a space of input-output systems, a generalized adjoint map from this space of systems to the space of generalized adjoints is defined. Then properties of the generalized adjoint map are derived. Afterward the author demonstrates that the inverse of an input-output system may be represented in terms of the generalized adjoint. The use of generalized adjoints to determine bounds for undesired inputs such as noise and disturbance to an input-output system is presented and methods which parallel adjoints in linear systems theory are utilized.
