Nach den in jeweils zweijahrigen Inter…vallen vorausgegangenen Seminaren uber 'Bildgebende Diagnostik in der Urologie', 'Intravesikale Chemothera pie und transurethrale Verfahren zur Behandlung und Rezidivprophylaxe des Blasenkarzinoms' und 'Klinik und Nachweismethoden der Mikrohamaturie' hat sich bei Gesprachen und Diskussionen im letzten Sommer und wahrend des Deutschen Urologen-Kongresses in Mainz mit vielen Kollegen und vor allem den hier als Autoren vertretenen Mitgliedern des Onkologischen Arbeitskrei ses der Deutschen Gesellschaft fur Urologie eine Bestandsaufnahme der zyt ostatischen Therapiemoglichkeiten bei urologischen Tumoren als Seminar thema gewissermaBen aufgedrangt. Ein entsprechendes Seminar der Urologi schen Univ.-Klinik Mainz uber 'Chemotherapie urologischer Malignome' liegt ja immerhin schon fast 7 Jahre zuruck. Das Thema ist besonders deshalb von aktuellem Interesse, weil wir Urologen in Klinik und Praxis doch mehr und mehr mit Tumorpatienten konfrontiert werden. Beispielsweise in meiner 70-Betten-Klinik in Darmstadt machen die Patienten mit Prostata-, Blasen-oder Nieren-Karzinom inzwischen schon fast 30 % der Belegung aus. Diese zunehmende Zahl von stationar zu behandeln den Tumorpatienten hat inzwischen gewissermaBen auch einen berufspoliti schen Aspekt erhalten, weil Urologische Kliniken, die nicht die Moglichkeit ei ner extrakorporalen Steinzertrummerung anbieten konnen, wie beispielsweise meine Klinik in Darmstadt, die noch dazu im Umkreis von nur 50 km von 5 Ba dewannen eingerahmt ist, durch die nun fehlenden Steinpatienten einen deut lich spurbaren Belegungsruckgang hinnehmen muBten. 84 pp. Deutsch.

Da sich das Spektrum der endoskopischen Eingriffe beim Blasenkarzinom doch in letzter Zeit erheblich erweitert hat, erschien es zweckmaBig, den Stellenwert dieser konservativen ('blasenerhaltenden') Therapiemodalitaten zu tiberprtifen. 99 % ailer Blasentumoren sind primar epitheliale Tumoren und tiber 95% sind urothe lialen Ursprungs, weil gerade das Dbergangsepithel der Blase eine ausgepragte Prolife mtionstendenz aufweist: Hyperplasie, Dysplasie oder Atypie bis zum eigentlichen Carcinoma in situ kennzeichnen die Schritte der tumoralen Histogenese. Charakte ristisch ist das multifokale Vorkommen mit Beteiligung der gesamten Blasenschleim haut im Sinne der panurothelialen Erkrankung und die groBe Rezidivhaufigkeit von etwa 2/3 aller oberflachlichen Harnblasentumoren mit Progression von Infiltrations tiefe und Differenzierungsgrad als mbglicher Ausdruck der fortgesetzten Exposition des Urothels fUr im Urin ausgeschiedene Kanzerogene. FUr klinische BedUrfnisse sei deshalb die Annahme erlaubt: Blasentumor ist gleich Malignom; denn etwa 50% der Patienten mit einem 'Papillom' sterben im Verlauf von 15 Jahren am fortgeschritte nem Blasenkarzinom. Daher wird im TNM-System die Bezeichnung T , Go-Urothel A karzinom verwendet. 96 pp. Deutsch.

Nun konnen wir die Beitrage und Diskussionen der dritten Fortbildungsveranstaltung in Darmstadt im November 1982 'Klinik und Nachweismethoden der Mikrohamaturie' in gedruckter Form vorlegen. Der Firma Madaus-Diagnostik Koln danke ich fUr die Organisation dieser Veranstaltung und die Obemahme aller Kosten. Schon im 11. Jahrhundert hielt man ffir die sogenannte Uroskopie, d.h. den Nachweis der Hamaturie, den Morgenurin am geeignetsten, da man der Auffassung war, d~ wiihrend des Schlafes das ganze Wesen des Menschen im Harn zuriickgehalten und so mit nicht durch die 5 Sinne verausgabt wurde. Da ffir den mittelalterlichen Arzt alles bedeutungsvoll war - zumal es belanglose Zuflilligkeiten im gottlichen Kosmos nicht geben durfte -, hatte ihm keine Struktur des Urins, weder die Fiirbung noch die Kon sistenz zu entgehen. Die damalige Medizin sah ihren Kosmos sozusagen im Hamglas. Somit hatte die Uroskopie bereits im Mittelalter eine derart beherrschende Stellung er langt, d~ sie zur wichtigsten iirztlichen Untersuchung, das Uringlas gewisserm~n zum einzigen Sprechzimmerinventar des Arztes wurde. Und da der Urin fUr das Abbild des ganzen Menschen gehalten wurde, brauchte der Arzt den Kranken noch nicht ein mal zu sehen, urn eine Diagnose oder Prognose zu stellen. 45 pp. Deutsch.
