Parallel dazu hat sich unser Wissen zur speziellen Anatomie dieses Gelenkes, Pathophysio logie, Diagnostik und Therapie der Traumafolgen wesentlich… erweitert. Weltweit ist man sich heute einig, daB eine Bandverletzung mit Instabilitat eine anatomische Rekonstruk tion verlangt. Durch dieses Vorgehen kann in frischen Fallen eine Erfolgsquote von urn 90% erreicht werden. Weit vorsichtiger ist die Prognose bei der chronischen Instabilitat zu stellen. Die iiberaus zahlreichen operationstechnischen Verfahren, die im Verlaufe der Jahre durch die Literatur gegangen sind, verraten eine gewisse Unsicherheit und Unzufriedenheit mit dem jeweils Erreichten. Wir selbst hatten mit ortsstandigem Bandersatz recht giinstige Friihergebnisse, muBten aber bei Spatkontrollen, wie viele andere, wieder eine groBere Zahl von Sekundar instabilitaten - wohl durch zunehmende Dehnung der Strukturen - feststellen. Seit iiber 8 Jahren beschaftigen wir uns deshalb in engem Kontakt mit Zentren in England (Jenkins), Frankreich (Lemaire), Siidafrika (Strover) und den USA (Alexander, Weiss) mit dem Bandersatz durch Kohlenstoffasern. Dieses Material besitzt hervorragende biomechanische Eigenschaften und eine auBer ordentlich hohe Biokompatibilitat, leider aber auch eine hohe Fragilitat bei Kriimmungiiber einen kleinen Radius. Die Beschichtung der Bander, ihre Einscheidung bei intraarticularer Anwendung und operationstechnische Besonderheiten vermogen die Rupturgefahr signi fikant zu senken. Wir sind weit davon entfernt, die Kohlenstoffaserbander als das heute einzig 'wahre' Material zu empfehlen, werden aber durch die festgestellten Dauerergebnisse ermutigt, auf diesem Gebiete weiterzufahren.Springer-Verlag GmbH, Tiergartenstr. 17, 69121 Heidelberg 172 pp. Deutsch.
