Braasch made several foundational contributions to the field of hepatopancreatobiliary (HPB) surgery, specifically focusing on refining, standardizing, and improving the safety of pancreaticoduodenal resection (the Whipple procedure). During his tenure as the Chairman of General Surgery at the Lahey Clinic, he became an influential pioneer in transforming what was once a highly lethal operation into a standardized, survivable procedure. His primary contributions to pancreaticoduodenal resection include: 1. Standardization and Advocacy for Lower Mortality. In the mid-to-late 20th century, pancreaticoduodenectomy was nearly abandoned by the medical community due to hospital mortality rates exceeding 25%. Dr. Braasch was vocal in defending the utility of the surgery. Through his landmark publications, such as his 1977 paper "Considerations That Lower Pancreatoduodenectomy Mortality," he proved that meticulous technique, precise patient selection, and localized institutional expertise could significantly lower perioperative mortality and safely offer patients a chance at a cure. 2. The Cattell-Braasch-Valdoni Maneuver. Alongside Dr. Richard Cattell, Dr. Braasch popularized a crucial surgical technique known as the Cattell-Braasch maneuver (later modified by Valdoni). This technique involves an extensive right-sided medial visceral mobilization and mobilization of the entire small bowel mesentery. In a Whipple procedure, performing this maneuver allows the surgical team to fully expose the retroperitoneal space and safely visualize the superior mesenteric artery (SMA) axis. This exposure is vital for the modern "mesopancreas-first" approach, allowing radical oncological clearance of pancreatic head tumors while safeguarding massive blood vessels. 3. Early Adoption and Validation of Pylorus Preservation. Dr. Braasch was an influential early adopter and major clinical validator of the pylorus-preserving pancreaticoduodenectomy (PPPD), originally introduced by Longmire and Traverso. Classic Whipple procedures required removing a portion of the stomach. Dr. Braasch published extensive clinical series (including a major 1986 review of 87 patients) demonstrating that preserving the pylorus and stomach resulted in satisfactory postoperative weight gain, eliminated debilitating post-gastrectomy digestive symptoms, and did not increase the risk of marginal ulcers. His work established PPPD as a preferred option for benign diseases and specific periampullary malignancies. 4. Education and Workforce Development. Beyond his technical innovations, Dr. Braasch structured the subspecialty itself. In 1981, he founded the Lahey Clinic Gastric, Hepatobiliary, and Pancreatic Surgical Fellowship. As one of only three such programs in the United States at the time, it directly trained a significant portion of the generation of surgeons who standardized complex pancreatic resections across North America.".
