"To refer to the epidemiology of opiate-addiction is not a figure of speech or a rationalization for the use of special techniques but a reflection of a literal, burgeoning reality: opiate-addiction and other forms of severe drug-abuse constitute an epidemic, a scourge, and even, in the slums of our big cities, a plague.".
