Der junge Arzt, welcher nach vollendetem Studium in die Praxis eintritt steht anfangs oft seinen Krankheitsfällen und insbesondere denen aus dem Gebiete der inneren Medizin ziemlich…unsicher gegenüber. Das liegt zum Teil daran, daß eine exakte Diagnosenstellung ohne die Hilfsmittel klinischer Beobachtung schwierig oder unmöglich ist, zum Teil auch wohl daran, daß der Arzt in der Praxis zu den Anfangsstadien der Erkrankungen gerufen wird ? und deswegen in der Tat viele Kranke mit erst wenig ausgesprochenen oder mehrdeutigen Symptomen sieht. Allein der Hauptgrund für die unbefriedigende Unsicherheit ist darin zu suchen, daß das klinische Denken, wenn dieser Ausdruck erlaubt ist, in der kurzen Zeit des Studiums nicht genügend geschult oder überhaupt erst in längerer Erfahrung ausgebildet werden kann. Um richtige Diagnosen zu stellen, genügt es nicht oder wenigstens in recht vielen Fällen nicht, daß der Arzt ein ausreichendes Schulwissen der Symptomatik besitzt und über eine genügende Untersuchungstechnik verfügt, sondern es ist nötig, daß er gelernt hat, rechtzeitig alle Möglichkeiten in Betracht zu ziehen, die im gegebenen Falle vorliegen können, daß er eben klinisch denken kann. Das ist keineswegs nur unerläßlich bei wirklich unklaren Fällen, bei denen darum handelt, unter verschiedenen Möglichkeiten die wahrscheinlichste zu erkennen_ also eine Wahrscheinlichkeitsdiagnose zu stellen; auch bei Erkrankungen die an sich einer rationellen Diagnose-zugänglich sind, bei denen die vorhandenen Erscheinungen einen bestimmten sicheren Schluß gestatten, ist dieses Denken an alle Möglichkeiten unbedingt erforderlich. Gewiß sollte eigentlich eine eingehende und gewissenhafte Untersuchung alle überhaupt feststellbaren Symptome finden, aber jeder erfahrene Arzt wird zugeben, daß nur der Untersucher alle Symptome richtig und vollständig feststellt, der weiß, daß er sie im einzelnen vorliegenden Falle erwarten darf und der deswegen sein besonderes Augenmerk auf die zu erwartenden Symptome richtet. Das tritt bekanntlich schon bei der Aufnahme der Anamnese klar zutage. An der Art, wie der Arzt die Anamnese aufnimmt, erkennt man seine klinische Erfahrung. Es wird nun vielfach behauptet, daß die Berücksichtigung der verschiedenen Möglichkeiten Sache des ?klinischen Scharfblickes" sei, einer besonders glücklichen ärztlichen Veranlagung, die intuitiv Wesentliches von Unwesentlichem zu sondern vermöge. Daran ist gewiß etwas Wahres, viele derartige Schlüsse mögen unterbewußt ablaufen. Aber ich glaube doch, daß diese Kunst in weitgehendem Maße durch die Erfahrung erlernt werden kann und dann auch bewußt ausgeübt wird. Eine weitere Schwierigkeit, die besonders bei der hausärztlichen Tätigkeit fr_ die Erscheinung tritt, mag gleichfalls Erwähnung finden.
