Bononiae : ex officina Emygdii ab Ulmo, 1840, 4°, (2), 10 pp., feine Broschur. Erstdruck! "C. B., Militairmusikus, 30 J. alt, wurde aus einem Cabriolet geworfen u. erlitt eine Fractur des linken Oberarms zugleich mit Luxation des Schultergelenkes. Bei der 3 Stunden darauf angestellten Untersuchung fand sich der Arm bereits um da…s Doppelte seines Umfanges geschwollen u. bei der leisesten Berührung schmerzhaft. Dagegen war die Gelenkpartie weder sehr geschwollen, noch schmerzhaft. Die Fractur war in der Quere u. im obern Dritttheile des Humerus erfolgt, die Luxation nach vorn, so dass der Gelenkkopf unter dem M. pectoralis zu fühlen war. Pat. ward ins Bett, der Arm in die möglichst passende Lage gebracht, Umschläge aus Essig u. Wasser (posca), wiederholte Aderlässe u eröffnendes Getränk bei sparsamer Kost verordnet. Hierbei verminderten sich alle die Fractur begleitenden Zufälle u. am 11. Tage ward, um die schon verminderte Geschwulst vollends zu beseitigen, um Finger, Vorderarm u. über die Hälfte des Oberarms eine Binde angelegt, zugleich aber durch mässige Extension die Fractur reponirt, u., um die Bruchenden in der gegebenen Lage zu erhalten, ausser einer leinenen Binde noch zwei Schienen von dicker Pappe vorn u. hinten an den Arm angelegt. Am 16. Tage nun versuchte Vf. die Luxation durch Einstemmen des Knies in die Achselhöhle, u. indem er den künstlich consolidirten Oberarm nach Art eines Hebels erster Gattung wirken liess, zu reponiren. Zuvor jedoch gab er dem Pat., um eine möglichste Muskelerschlaffung zu erzielen, u. so jenes immerhin gewaltsame Verfahren für den gebrochenen Arm möglichst unschädlich zu machen, eine kleine Dosis Tartarus stibiatus bis zum Uebelwerden. Dann zog er den schon vorher angelegten Verband fester an, u. legte an den vordern Theil des Arms noch eine aus zwei in etwas schiefem Winkel zusammenlaufenden Schienen gebildete Schiene an, um die Bewegungen des Vorderarms gegen den Oberarm dadurch zu verhüten. Er liess nun den Pat. aus dem Bette heben u. auf einem Sessel niedersitzen; stellte sich selbst hierauf ihm gegenüber, u. seinen rechten Fuss so auf den Sessel, dass sein Knie in die Achselhöhle des Pat. zu liegen kam. Ein Gehülfe begann nun mit dem am Verbande freien Theile des Oberarms zu extendiren, während ein andrer den Truncus des Pat. fixirte, jedoch ohne allen Erfolg. Vf. brachte jetzt sein Knie in die Achselhöhle des Pat. u. erhob den Arm ein wenig, so dass derselbe sich auf das Knie stützte u. einen Hebel erster Gattung darstellte, dessen Kraft (potentia) im Ellbogen, dessen Stützpunkt (fulcrum) im Knie gelegen war. Vf. drückte nun den Ellbogen gegen den Truncus an, u. war so im Stande, den Gelenkkopf zurückzubringen. Die Umstehenden hörten den eigenthümlichen Ton des Einschnappens u. glaubten den Gelenkkopf schon in die Gelenkhöhle zurückgekehrt. Die Untersuchung zeigte jedoch, dass derselbe zwar unter dem Pectoralis vor u. an den Glenoidalrand der Scapula sich begeben habe, jedoch noch nicht in die Gelenkhöhle zu Knie unter die Achselhöhle des Pat., erhob den Arm rückgekehrt sei. Vf. brachte deshalb abermals sein so weit, dass er eine fast horizontale Lage annahm, u. drückte ihn dann mit einiger Gewalt gegen sein Knie herab, wodurch der Gelenkkopf so nach aussen sprang, dass er der Gelenkhöhle gegenüber zu stehen kam u., sich selbst überlassen, alsbald in dieselbe einschnappte. Nachdem so die Luxation vollkommen gehoben war, wurden Schienen u. Bänder entfernt, u. es zeigte sich, dass der Arm durchaus nicht durch die auf ihn einwirkende Gewalt gelitten hatte. Auch in den folgenden Tagen erschienen keine Zeichen von Reizung oder Entzündung, vielmehr heilte die Fractur chartis crassis) bald u. so vollständig, dass Pat. seiunter Erneuerung des Pappverbandes (apparatus ex nen Arm mit derselben Gelenkigkeit beim Handhaben seines Instrumentes (Clarinette) wieder brauchen lernte. Sein Ruf war bereits so ausgebreitet, das er vielfach zu Consultation nach Rom und 1836 zum Pabst Gregor XVI. dahin als General Director des Militär-Sanitätsdienstes berufen und später von Pius IX. zu seinem Leibchirurgen ernannt wurde. Da er während der ephemeren römischen Republik, 1849, den Verwundeten hilfreich beigestanden hatte, fiel er nach Wiederaufrichtung der päpstlichen Gewalt beim Vatican in Ungnade, und wurde aller seiner Aemter entsetzt, was er mit Gleichmuth ertrug. Jedoch war auch seine Gesundheit, auch in Folge häuslichen Unglücks, erschüttert und starb 2.April 1854. - Er war einer der ausgezeichnetsten ital. Chirurgen, namentlich als Operateur; er gehörte zu den Ersten, welche in Italien plastische Operationen und die Lithotripsie ausführten, ebenso die Exarticulation im Hüftgelenk, die Resectionen des Schulterblattes, des Oberkiefers, des Brustbeines, der Rippen, ferner die Neurectomieen u.s.w. Er hat nur wenig publicirt, darunter besonders einige Abhandlungen über die "Autoplastica" und die "Litotrizia"." Paulus Baroni (1799-1854) was an outstanding Italian surgeon and professor of surgery and obstetrics at Cologna; called to Rome in 1836 by Pope Gregory XVI as Director of Health in the military service. Personal physician to Pius IX. Distinguished himself for some of his procedures in plastic surgery; one of the first to practice lithotripsy, and, according to Hirsch, the first in Italy.
