Gebraucht, aber gut erhalten. - Von großem praktischem Interesse ist für den Chirurgen die Frage, wie man die immunologische Reaktion eines Körpers soweit zurückdrängen kann, daß Organtransplantationen von einem Individuum auf ein anderes mit kl…inischem Erfolg durchgeführt werden können. Hier ergeben sich theoretisch vier Möglichkeiten: 1. Man kann am Spender ansetzen und versuchen, die antigenen Eigenschaften des gespendeten Organes so auszuwählen, daß sie zu dem Empfänger passen. Eine solche Spenderwahl ist heute auf verschiedene Weise möglich: Die Mindestvoraussetzung bei der Spenderwahl ist die Verträglichkeit der Blutgruppen. Die gegenwärtige Gewebstestung beruht im wesentlichen auf 2 Verfahren: Bei der Bestimmung der Gewebsgruppen, genauer der Leukozytenantigen-Grup-pen - die man Typing nennt - versucht man die bisher bekannten Trans-plantationsantigene serologisch zu erfassen, um einen gegebenen Empfänger nach seiner geweblichen Stellung in einem bestimmten Kollektiv von möglichen Spendern zu lokalisieren (Abb. 6). Von einer idealen Gewebsgruppenbestimmung ist man allerdings noch weit entfernt, da nicht alle Gewebsantigene bekannt sind und serologisch erfaßt werden können. Die Erkenntnisse auf diesem Gebiet machen aber große Fortschritte. Bei dem 2. Testverfahren, der gemischten Lymphozytenkultur (Abb. 7), dem sogenannten Matching, wird aus der Reaktion der sowohl antigentragenden, wie auf Fremdantigen reagierenden Spender- und Empfängerlymphozyten in der Zellkultur auf die gewebliche Ähnlichkeit bzw. Changing times bring renewed problems with infectious diseases. Ausreichende Kenntnisse über die technischen Möglichkeiten dieser Therapie, die Auswirkungen ihrer einzelnen B…austeine auf den Stoffwechsel des Kran ken sowie ihre Risiken sollten zum Rüstzeug jedes klinisch tätigen Arztes gehören. Es kann heute keine Entschuldigung mehr dafür geben, daß der Nahrungsbedarf eines Kranken ungedeckt bleibt, nur weil er nicht mehr in der Lage ist, oral genügend Nah rung aufzunehmen. Die Standardinfusionstherapie, üblicherweise bestehend aus 2 bis 3 Liter Elek trolyt-Aminosäuren- oder niedrigprozentigen Kohlenhydratlösungen unterscheidet sich von der hochkalorischen parenteralen Ernährung vor allem durch die Osmolarität des Infusionsgemisches und dem Zugangsweg. Bei der Standardtherapie ist der peri phere Zugang die Regel und ein Verlust an Struktur- oder Funktionsproteinen wird bewußt in Kauf genommen, da eine ausreichende Energiezufuhr zur Verstoffwechse lung der zugeführten Aminosäuren nicht möglich ist, ohne Schäden an der Venen wand hervorzurufen. Durch die hochkalorische parenterale Ernährung werden, wobei der zentrale Venenkatheter eine Grundvoraussetzung ist, dem Kranken hochprozen tige Kohlenhydratlösungen, Fette und Aminosäuren in einer Größenordnung verab reicht, die nicht nur seinen aktuellen Bedarf decken, sondern auch vorbestehende Mangelzustände ausgleichen können. Die Kathetertechnik und die Entwicklung der Infusionslösungen befindet sich auf einem so hohen Stand, daß eine parenterale Versorgung des Organismus mit Nährstoffen auch über Monate hinweg möglich ist. Besondere Beobachtungen erfuhr in derletzten Zeit die Anwendung der hoch kalorischen parenteralen Ernährung bei den entzündlichen Darmerkrankungen und in der Onkologie.
