Mit 70 teils farbigen Abbildungen und 32 Tabellen. Die primär biliäre Zirrhose kennzeichnet nur das zirrhotische Spätstadiuum einer chronischen, nicht eitrigen, nicht bakteriellen Cholangitis. Es ist eine seltene Erkrankung mit einer jährlichen Neuinfektionsrate von ca. 5:100.000 Personen. Meist sind Frauen jenseits des 40. Lebensjahr betroffen. Oft bleiben die Patienten jahrelang asymptomatisch, d.h. sie fühlen sich wohl ohne jeglichen Krankheitssymptome, können jedoch durch zufällig dokumentierte Leberwerterhöhungen der ALP und GGT auffallen. Erst später kommen ein quälender Juckreiz, Müdkigkeit, eine Vergrösserung der Leber oder extrahepatischen Symptome wie eine Schilddrüsenentzündung, oder rheumatoide Gelenksbeschwerden hinzu. Unbehandelt kann sich auch hier eine Leberzirrhose mit all ihren Folgen entwickeln. Typische Laborzeichen sind Erhöhungen der anitmitochondrialen Antikörper (AMA) in über 90% der Fälle, eine starke IgM erhöhung, Erhöhung der ALP, GGT und des Bilirubins. Eine ursächliche therapie ist nicht bekannt, jedoch beeinflußt Ursodeoxycholsärue den Krankheitsverlauf in den Frühstadien ( I - III) sehr günstig in einer Dosierung von 15 mg/kg Körpergewicht. Sollte es zur Ausbildung einer fortgeschrittenen Leberzirrhose kommen, so kann bei einem Bilirubinspiegel von ca. 5 mg % eine Lebertransplantation erwogen werden, da in diesem Stadium die Lebenserwartung doch deutlich eingeschränkt ist. Die primär sklerosierende Cholangitis ist ebenfalls eine seltenen Erkrankung mit einer jährlichen Neuinfektionsrate von 1-5:100.000 Personen. Hiervon sind vor allem Männer im Alter zwischen 30 und 50 Jahren betroffen. In mehr als als 70% der Fälle liegt gleichzeitie eine chronisch entzündliche Darmerkrankung (Colitis ulcerosa) vor. Im Frühstadium bleibt die oft Erkrankung unerkannt, da die Patienten keine Beschwerden haben, später kommen eine Gelbsucht und ein Juckreiz hinzu. Eine Leberzirrhose mit all ihren Folgen kann sich bei unbehandelten oder sehr spät diagnostizierten Patienten entwickeln. Laborchemisch ist die Erkrankung durch eine Erhöhung der ALP und GGT gekennzeichnet. Die Diagnostik der Wahl ist die endoskopisch retrograde Cholangiographie, die Gallenwegsspiegelung und Darstellung mit Kontrastmittel (ERC). Bei der Therapie haben sich die Ursodeoxycholsäure in einer Dosis von 15 mg/kg Körpergewicht und bei führenden Engstellen der großen Gallengänge wiederholte endoskopische Aufdehnungen (Dilatationen) der Gallengänge etabliert. Hierdurch werden Lebensverlängerungen und Verlängerung der Zeiten bis zu einer möglicherweise notwendigen Lebertransplantation erreicht.
