ABOUT THE BOOK: This book dwells on only one factor in some depth, i.e., the availability of toilets and the disposal of human excreta by building toilets in the context of equity. It discusses the availability of toilet facility at the household level in India by examining the available information on households having toilets over time and variations across regions. Further, it deals with the health impact of improper disposal of or handling of human excreta, especially that of young children. Next, it examined the sociological, cultural and economic factors hindering the use of toilets and the consequences of open defecation especially on women. The book discusses the role of caste and efforts to restore human dignity among and rights of the scavenging community and it also gives a brief discussion on the design of individual toilets and community complexes such that the need to handle excreta by humans is eliminated. The book raises some questions for further reflection and research. ABOUT THE AUTHOR: Leela Visaria holds PhD from Princeton University and is a former Professor and Director of GIDR. Currently she is an Honorary Professor at the Institute. Her research interests include historical demography as well as fieldbased studies on problems of health, family planning, education and demographic transition. Her articles have appeared in several scholarly journals. She has been a coauthor of a book Contraceptive Use and Fertility in India: A Case Study of Gujarat (Sage Publications, New Delhi, 1995) and of India in the 21st Century along with Tim Dyson and Robert Cassen, both of London School of Economics (Oxford University Press, 2004). She has coedited (withJohn Simons and Peter Berman) Maternal Education and Child Survival: Pathways and Evidence (Vikas Publishing House, Delhi, 1997); Abortion in India: Ground Reality (jointly with Vimala Ramachandran), New Delhi, Routledge, 2007; and Midwifery & Maternal Health in India: Situation Analysis & Lessons from the Field, CMHS Monograph No.1, Indian Institute of Management, Ahmedabad, 2010. This book fills a gap in our understanding of the ground realities with respect to induced abortion in India to create an evidence-based body of knowledge. Using both quantitative and qualitative research methods, the case studies show why and under what circumstances women seek abortion and the quality of services available to them. They also explore inter-generational differences in attitudes and practices, the perceptions and selection of providers, female-selective abortion, and informal abortion practitioners. Among other issues, the contributors show that strong preference for sons, availability of modern techniques for diagnostic tests, widespread acceptance of the small family norm, and heavy reliance on female sterilisation as the primary method of contraception lead women to abort unwanted pregnancies. A book that goes beyond the smokescreen of data and regulations to unravel the human story behind elective abortion, it will be of interest to those studying health, public policy, and gender, apart from the general reader.

In the early 2000s, a lack of reliable information, wide regional and rural-urban differences and a thin research base made it difficult for policymakers, administrators and women's health advocates to develop strategic interventions. The earlier edition of this book used the in-depth qualitative research studies done in the early 2000s to highlight issues that emerged. These studies were part of the Abortion Assessment Project - India (AAP-I) jointly anchored by the Centre for Enquiry into Health and Allied Themes (CEHAT) and HealthWatch. The studies included sought to build a body of knowledge on all facets of induced abortion, such as the unmet needs for contraception and abortion, the son preference, the preference for private providers and the neglected needs of the single, widowed or separated women, to list policy-level and on-ground implementation issues. More crucially, they highlight the need to integrate diverse viewpoints that would further ease progression towards the common goal of making abortion an infrequently used but safe alternative for women faced with an unwanted pregnancy. The second edition revisits the studies in light of demographic shifts, changing social attitudes, technological advances and expanded access to medical abortion, reflecting evolving reproductive rights in India.
