Hutton includes excerpts from letters, passages from diaries, and photographs in her account of the five and a half years she spent in the Scottish Women's Hospitals for Foreign Service during the First World War. Isabel Emslie (1887-1960), who later became Lady Hutton, was a Scottish physician specializing in mental disorders and a social worker. In August 1914, she joined Dr Elsie Inglis's Scottish Women's Hospitals for Foreign Service, a voluntary organization serving France, Russia, Greece, Corsica and Serbia, in almost all-female units, providing doctors, nurses, drivers, administrators, orderlies and cooks. In recognition of her work in this era, she was awarded the Serbian orders of the White Eagle and St. Sava, the French Croix de Guerre, and the Order of St. Anna of Russia. With a Woman's Unit was praised in the Times for its "accounts of interesting Serb and Russian customs and ceremonies" and "very shrewd observations on matters political and economic".
