Er schrieb u. a. die Oper Dr. Eisenbart' (1922), vier Symphonien, Konzerte, Kammermusik, Chorwerke sowie Musik für Akkordeon. Aufgrund der therapeutischen Wirkung seiner Musik wurde [ihm] 1926 die Ehrendoktorwürde der Medizinischen Fakultät der Universität Würzburg verliehen" (DBE). Seit 1908 als Professor an der Akademie der Tonkunst in München, war Zilcher von 1920 bis 44 Direktor des Staatskonservatoriums in Würzburg und Leiter einer Meisterklasse für Komposition.
