There is the original jacket, this only has a very small scuff on the bottom of the spine section. 'Published on the occasion of the exhibition Eyes, Lies and Illusions, Hayward Galler…y, London, 7 October 2004 - 3 January 2005'. There is a pictorial illustration with detailed eyes on the frontispiece. The pages are like new. Included is the original acetate that is used over the top of the patterns to create additional illusions (P88). There is also a silver card template which is stored in a flap on the back board. I would have classed the overall book as Fine, if it didn't have the small scuff on the jacket.
